摘要
目的比较微血管减压术(MVD)与经皮三叉神经半月节射频热凝术(PRT)治疗MVD术后复发三叉神经痛疗效.方法回顾性分析2013年1月至2019年1月138例MVD术后复发三叉神经痛患者的临床资料,分为MVD组与PRT组,比较两组患者的术后疼痛治疗有效率、复发率及术后并发症发生率等.结果MVD组和PRT组有效率分别为93.7%和90.7%,两组比较差异无统计学意义(P>0.05);MVD组治愈率高于PRT组(85.7%vs 45.3%,P<0.05);PRT组复发率高于MVD组(54.7%vs 14.3%,P<0.05);MVD组手术时间及住院时间长于PRT组[(98.6±26.5)min vs(24.7±11.8)min,(9.8±2.4)d vs(3.6±1.2)d,P<0.05];PRT组面部麻木发生率高于MVD组(66.7%vs 9.5%,P<0.05);MVD组与PRT组口角疱疹发生率比较差异无统计学意义(25.4%vs 29.3%,P>0.05).结论MVD和PRT治疗MVD术后复发三叉神经痛均有效.MVD治愈率更高,复发率低,面部麻木等并发症少等;而PRT手术时间和住院时间短,相较于MVD安全性更高、创伤更小.
Objective To explore the clinical effect of microvascular decompression(MVD)and percutaneous radiofrequency thermocoagulation(PRT)for recurrent trigeminal neuralgia after MVD.Methods The clinical data of patients with recurrent trigeminal neuralgia treated with MVD(30 cases,MVD group)and PRT(56 cases,PRT group)from January 2013 to January 2019 were analyzed retrospectively.The eficacy,recurrence rate nd postoperative complications of the two groups were compared.Results The effective rate of MVD group and PRT group was 93.7%and 90.7%respectively,there was no significant difference between the two groups(P>0.05);the cure rate of MVD group was higher than PRT group(85.7%vs 45.3%,P<0.05);the recurrence rate of PRT group was higher than MVD group(54.7%vs 14.3%,P<0.05);the operation time and hospitalization ime of MVD group were longer than PRT Group[(98.6+26.5)minvs(24.7+11.8)min,(9.8+2.4)d vs(3.6+1.2)d].The incidence of acial numbness in PRT group was higher than that in MVD group(66.7%vs 9.5%,all P<0.05).There was no statistic aly significant difference in the incidence of oral herpes between the MVD group and the PRT group(25.4%vs 29.3%,P>0.05).Conclusion Both MVD and PRT are effective in.the treatment of recurrent trigeminal neuralgia after MVD.MVD has the advantages of high cure rate,low recurrence rate,less facial numbness and other complications,while PRT has shorter operation time and hospital stay,higher safety and less trauma compared with MVD.
