摘要
目的分析远隔节段跳跃型胸椎管狭窄症一期手术与分期手术效果观察。方法88例远隔节段跳跃型胸椎管狭窄症患者,根据患者具体病情分为一期手术组和分期手术组,每组44例。两组均给予后路胸脊髓减压手术,一期手术组行一期手术治疗,分期手术组行分期手术治疗。比较两组患者手术前后日本骨科协会评估治疗分数(JOA)评分、生活质量评分以及术后并发症发生情况。结果一期手术组手术前后JOA评分分别为(4.65±1.21)、(8.31±1.03)分,与分期手术组的(4.23±1.35)、(8.47±2.11)分比较,差异均无统计学意义(P>0.05)。两组术后并发症发生率比较,差异无统计学意义(P>0.05)。一期手术组手术前后生活质量评分分别为(52.74±12.25)、(75.51±15.74)分,与分期手术组的(51.25±11.61)、(76.13±16.48)分比较,差异均无统计学意义(P>0.05)。结论临床对远隔节段跳跃型胸椎管狭窄症患者可实施一期手术及分期手术,但对于身体状态较佳及手术操作程度较低患者,可选用一期手术;而对身体状态欠佳及手术操作程度较高患者,多建议行分期手术。
Objective To analyze the effect of one-stage operation and staging operation for remote segmental jump thoracic spinal stenosis.Methods A total of 88 patients with remote segmental jump thoracic spinal stenosis were divided into one-stage operation group and staging operation group according to condition of disease,with 44 cases in each group.Both groups received posterior thoracic spinal cord decompression surgery.One-stage operation group received one-stage operation,and staging operation group received staging operation.The Japanese Orthopaedic Association(JOA)score and quality of life score before and after operation,and occurrence of postoperative complications were compared between the two groups.Results The JOA score of one-stage operation group before and after operation were(4.65±1.21)and(8.31±1.03)points,which had no statistically significant difference compared with(4.23±1.35)and(8.47±2.11)points of staging operation group(P>0.05).There was no statistically significant difference in incidence of postoperative complications between the two groups(P>0.05).The quality of life before and after operation of one-stage operation group were(52.74±12.25)and(75.51±15.74)points,which had no statistically significant difference compared with(51.25±11.61)and(76.13±16.48)points of staging operation group(P>0.05).Conclusion One-stage operation and staging operation can be performed for patients with remote segmental jump thoracic spinal stenosis.However,for patients with better physical condition and low degree of surgical operation,one-stage operation can be selected;for patients with poor physical condition and high degree of surgical operation,staging operation is often recommended.
