摘要
患者女,49岁,右侧乳晕出现黄豆大小浅褐色肿物10个月余,伴局部轻度瘙痒。右侧乳晕肿物组织病理示:真皮层可见多个团巢样结构,由相互交织增生的平滑肌细胞束组成,细胞大小形态均匀,核呈雪茄状,核周有空泡,未见明显异型性和有丝分裂象增加,未见坏死及钙化,病变周围胶原纤维增生,深部肌束间可见扩张或呈裂隙状的毛细血管及静脉。免疫组织化学示:ɑ-SMA(+),Desmin(+),CD34(血管+),Ki-67(1%+)。特殊染色:Masson(+)。诊断:乳晕平滑肌瘤。治疗:手术切除并随访。
A 49-year-old female presented with a soybean-sized light brown mass in the right areola for more than 10 months,with mild local itching.The histopathology of the right areola mass showed that there were multiple clusters of nest-like structures in the dermis,composed of interwoven and proliferating smooth muscle cell bundles.These cells had uniform size and morphology,cigar-shaped nuclei,perinuclear vacuoles,but no obvious atypia,necrosis and calcification could be seen.There were proliferated collagen around the lesions,and dilated or fissured capillaries and veins appeared between deep muscle bundles.Immunohistochemical staining showed:ɑ-SMA(+),Desmin(+),CD34(vascular+),Ki-67(1%+).Special staining:Masson(+).She was diagnosed by areola leiomyoma.Treatment of surgical excision and followed-up were given.
