摘要
目的:探究表观扩散系数(ADC)及相对表观扩散系数(rADC)值在不同时期缺血性脑梗死早期诊断中的应用价值。方法:回顾性分析230例缺血性脑梗死患者临床资料,根据患者出现临床症状到磁共振成像(MRI)检查时间分为超急性期(发病<6h)50例,急性期(6~24h)81例、亚急性期早期(24h~7天)69例和慢性期(>7天)30例。计算各区域的ADC、rADC值差异。分析脑梗死出血性转化(HT)。结果:不同时期患者脑梗死核心区、IP1区、IP2区的ADC值与rADC值差异均有统计学意义(P<0.05);而镜像区、IP1镜像区、IP2镜像区的ADC值差异均无统计学意义(P>0.05)。缺血性脑梗死发病时间与ADC、rADC值水平呈正相关(P<0.05)。HT组与NHT组rADC值下降程度的所占比例分布具有显著差异(P<0.05)。HT组ADC最低值和ADC平均值均显著低于NHT组(P<0.05)。结论:ADC值与rADC值应用于缺血性脑梗死早期诊断具有良好效能,其水平随发病时间延长呈现相关性变化,提示梗死病灶变化和出血风险,对治疗方案选用及预后预测具有指导作用。
Objective:To explore the application value of apparent diffusion coefficient(ADC)and relative apparent diffusion coefficient(rADC)values in the early diagnosis of ischemic cerebral infarction in different periods.Methods:The clinical data of 230 patients with ischemic cerebral infarction were retrospectively analyzed.According to the time from the onset of clinical symptoms to the magnetic resonance imaging(MRI)examination of the patients,they were divided into 50 cases in the hyperacute phase(onset<6h)and acute phase(6-24h).81 cases,69 cases in the early subacute phase(24h-7days)and 30 cases in the chronic phase(>7days).Calculate the difference of ADC and rADC values in each area.Analyze hemorrhagic transformation(HT)of cerebral infarction.Results:The ADC value and rADC value of the core area,IP1 area and IP2 area of patients with cerebral infarction in different periods were significantly different(P<0.05);while the ADC value of the mirror area,IP1 mirror area and IP2 mirror area had no difference Statistical significance(P>0.05).The onset time of ischemic cerebral infarction was positively correlated with ADC and rADC values(P<0.05).There was a significant difference in the proportion distribution of rADC value decline between the HT group and the NHT group(P<0.05).The lowest ADC value and the average ADC value of the HT group were significantly lower than those of the NHT group(P<0.05).Conclusion:ADC value and rADC value have good performance in the early diagnosis of ischemic cerebral infarction,and their levels show correlation changes with the onset of time,suggesting changes in infarct focus and bleeding risk,and guiding the selection of treatment options and prognosis prediction.
