摘要
目的探讨Teach-back教育模式在COPD患者健康教育过程中应用的临床价值。方法采用方便抽样法,选择湖州市中心医院2018年3月-2019年3月收治的100例COPD患者为研究对象,随机分为对照组和研究组,每组50例。对照组采用常规健康教育,干预组在使用常规健康教育的基础上采用Teach-back教育模式。比较分析两组患者干预前后的COPD评估测试(CAT)评分、血气指标、肺功能指标以及出院后3个月的病情复发和再入院情况。结果干预后,研究组的CAT评分低于对照组,血气分析指标二氧化碳分压(PaCO_(2))、血氧分压(PaO_(2))以及血氧饱和度(SaO_(2))水平均优于对照组,第1秒用力吸气量(FEV1)、第1秒用力呼吸量与用力肺活量比值(FEV_(1)/FVC)水平均高于对照组,差异有统计学意义(P<0.01);出院3个月后,研究组的病情发作次数少于对照组,再次入院率低于对照组,差异有统计学意义(P<0.05)。结论COPD患者在健康教育中使用Teach-back教育模式能够改善患者的呼吸道症状,提高肺功能、PaO_(2)和SaO_(2),减少患者病情的复发和再入院,优化患者的生活质量,值得临床推广使用。
Objective To explore the clinical value of application of Teach-back education model in the process of health education for patients with chronic obstructive pulmonary disease(COPD).Methods Using the convenient sampling method,a total of 100 COPD patients admitted to Huzhou Central Hospital from March 2018 to March 2019 were selected as the research objects.They were randomly divided into the control group and the study group,with 50 cases in each group.The control group adopted conventional health education,while the intervention group adopted Teach-back education model on the basis of conventional health education.COPD Assessment Test(CAT)scores,blood gas indexes,lung function indexes of patients of the two groups before and after the intervention,as well as the recurrence and readmission status were compared and analyzed 3 months after discharge.Results After intervention,CAT score of the study group was lower than that of the control group,blood gas analysis indexes such as partial pressure of carbon dioxide(PaCO2),partial pressure of oxygen(PaO2)and blood oxygen saturation(SaO2)were better than those of the control group,and forced expiratory volume in one second(FEV1)and forced expiratory volume in one second/forced vital capacity(FEV1/FVC)levels were higher than those of the control group,and the differences were statistically significant(P<0.05).Three months after discharge,the frequency of disease attacks in the study group was less than that in the control group and the readmission rate was lower than that in the control group,and the differences were statistically significant(P<0.05).Conclusions The use of Teach-back education model in health education for COPD patients can significantly improve the patients'respiratory symptoms,improve lung function,PaO2 and SaO2,reduce the recurrence and readmission of patients and optimize quality of life of patients,which is worthy of clinical promotion and application.
