摘要
目的采用加权TOPSIS-RSR模型法综合评价某三甲医院住院医疗服务质量,为临床科室精细化管理和综合考核提供科学依据。方法选取DRGs中反映医疗服务能力、运营效率和医疗安全的7个相关指标,运用加权TOPSIS-RSR模型对2019年1月1日-2019年12月31日某三甲综合医院23个临床科室的住院服务质量进行评价。结果根据回归方程估计值将23个临床科室的住院服务质量拟分为4档,方差分析和比较均显示4组的住院服务质量差异显著(F=111.21,P<0.001)。其中心血管科(C_(i)=0.576)、心脏大血管外科(C_(i)=0.550)、骨科(C_(i)=0.540)、普通外科(C_(i)=0.519)为优档;神经内科(C_(i)=0.398)、消化内科(C_(i)=0.373)、呼吸内科(C_(i)=0.363)、胸外科(C_(i)=0.353)、泌尿外科(C_(i)=0.352)为良档,肿瘤放疗科(C_(i)=0.218)和肿瘤科(C_(i)=0.212)为差档,其余科室为中档(0.341≥C_(i)≥0.247)。结论基于DRGs数据的加权TOPSIS-RSR模型评价结果可靠,住院医疗服务质量存在科室差异,科室应该从医疗服务能力、运营效率和医疗安全等方面对标找差距,针对薄弱环节,采取相应措施不断改善医疗服务质量。
Objectives The weighted TOPSIS-RSR model was used to evaluate the quality of inpatient medical service in a Three A and Tertiary hospital,and provide scientific basis for fine management and comprehensive assessment of clinical departments.Methods 7 relevant indicators reflecting medical service capacity,operational efficiency and medical safety in DRGs were selected and using the weighted TOPSIS-RSR model to evaluate the quality of inpatient services of 23 clinical departments in a top three hospital on January 1st,2019 solstice December 31st,2019.Results According to the estimated value of regression equation,the inpatient service quality of 23 clinical departments was divided into 4 grades,anova and pairwise comparison showed significant differences in the quality of in-patient service among the four groups(F=111.21,P<0.001).Department of cardiology(C_(i)=0.576),cardiovascular surgery(C_(i)=0.550),orthopedics(C_(i)=0.540),general surgery(C_(i)=0.519)were the best grade,neurology(C_(i)=0.398),gastroenterology(C_(i)=0.373),respiratory medicine(C_(i)=0.363),thoracic surgery(C_(i)=0.353)and urology(C_(i)=0.352)were divided into the second grade,the poor grade were department of tumor radiotherapy(C_(i)=0.218)and oncology(C_(i)=0.212),and other departments for middle grade(0.341≥C_(i)≥0.247).Conclusions The weighted TOPSIS RSR model based on DRGs data was reliable,and there were department differences in the quality of inpatient medical service.Departments should find out the differences from the aspects of medical service capacity,operational efficiency and medical safety,and take corresponding measures to improve the quality of medical services.
