摘要
目的:观察益肾汤联合科素亚治疗肾阴亏虚型2型糖尿病肾病(DKD)的临床疗效。方法:将肾阴亏虚型2型DKD患者62例,随机分为治疗组和对照组各31例。对照组口服科素亚(氯沙坦钾片)治疗,治疗组在对照组的基础上给予益肾汤治疗,2组均连续治疗8周。观察2组治疗前后的尿微量白蛋白/尿肌酐比值(ACR)、24小时尿白蛋白定量(24 h-UTP)、尿素氮(BUN)、血肌酐(SCr)、糖化血红蛋白(HbA1c)、血管细胞黏附分子-1(VCAM-1)、细胞间黏附分子-1(ICAM-1)等水平变化。结果:治疗前,2组SCr、BUN、HbA1c、ACR、24 h-UTP、VCAM-1、ICAM-1水平比较,差异无统计学意义(P>0.05)。治疗后,2组SCr、BUN水平以及对照组HbA1c水平与治疗前比较,差异均无统计学意义(P>0.05);治疗组HbA1c水平较治疗前降低(P<0.05),且治疗组低于对照组(P<0.05)。2组ACR、24 h-UTP、VCAM-1、ICAM-1水平均较治疗前降低(P<0.05),且治疗组ACR、24 h-UTP、VCAM-1、ICAM-1水平均低于对照组(P<0.05)。结论:益肾汤联合科素亚治疗肾阴亏虚型2型DKD,可提高疗效,改善临床症状,降低24 h-UTP、ACR、VCAM-1、ICAM-1、HbA1c水平。
Objective:To observe the clinical effect of self-made Yishen tang combined with Cozzar for type 2 diabetic kidney disease(DKD)of kidney yin deficiency type.Methods:A total of 62 cases of patients with type 2 DKD of kidney yin deficiency type were randomly divided into the treatment group and the control group,31 cases in each group.The control group was given oral administration of Cozzar(losartan potassium tablets),and the treatment group was additionally treated with Yishen tang based on the treatment of the control group.Both groups were continuously treated for eight weeks.Changes in levels of urine albumin to creatinine ratio(ACR),24-hour urine total protein(24 h-UTP),blood urea nitrogen(BUN),serum creatinine(SCr),glycosylated hemoglobin A1 c(HbA1 c),vascular cell adhesion molecule 1(VCAM-1),and intercellular adhesion molecule-1(ICAM-1)before and after treatment were observed in the two groups.Results:Before treatment,when compared levels of SCr,BUN and HbA1 c between the two groups,there was no significance in the difference(P>0.05).The comparison of levels of SCr and BUN of the two groups before and after treatment and the comparison of HbA1 c level in the control group before and after treatment showed no significance in differences(P>0.05).The level of HbA1 c in the treatment group was decreased when compared with that before treatment(P<0.05),and the level in the treatment group was lower than that in the control group(P<0.05).Before treatment,when compared levels of ACR,24 h-UTP,VCAM-1 and ICAM-1 between the two groups,there was no significance in the difference(P>0.05).After treatment,levels of ACR,24 h-UTP,VCAM-1 and ICAM-1 in the two groups were decreased when compared with those before treatment(P<0.05),and the four levels in the treatment group were lower than those in the control group(P<0.05).Conclusion:In the treatment of type 2 DKD of kidney yin deficiency type,the therapy of Yishen tang combined with Cozzar can enhance curative effect,improve clinical symptoms,and reduce levels of 24 h-UTP,AC
