摘要
目的分析不同麻醉方法在异位妊娠腹腔镜手术中的应用效果及安全性。方法在我院2019年7月—2020年7月期间收治的需要采用腹腔镜手术治疗的异位妊娠患者中,随机抽取100例展开研究,通过双盲选法分为甲组和乙组(各50例),分别予以气管插管麻醉和喉罩全麻。对2组患者麻醉期间各时间段的MAP、HR、PAW以及PETCO2变化进行观察,并且比较其麻醉安全性以及不同时间段的疼痛评分。结果诱导前及气腹后2组患者的MAP与HR指数比较,差异无统计学意义(P>0.05);乙组置喉罩前、喉罩时、喉罩后5 min、气腹前的MAP和HR波动均小于甲组,差异有统计学意义(P<0.05)。甲组并发症发生率显著高于乙组,差异有统计学意义(P<0.05)。组间不同时间段的疼痛评分比较,差异均无统计学意义(P>0.05)。2组患者气腹后5 min以及气腹后15 min的PAW以及PETCO2均较气腹前有显著升高,但组间比较差异无统计学意义(P>0.05)。结论腹腔镜手术治疗的异位妊娠患者,喉罩全麻对手术期间血流动力学的影响更小,麻醉安全性更高,可以在更大程度上减少并发症的发生。
Objective To analyze the effect and safety of different anesthesia methods in laparoscopic surgery for ectopic pregnancy.method In our hospital from July 2019 to July 2020,100 cases of ectopic pregnancy were randomly selected and divided into group A and group B(50 cases in each group)by double-blind selection.Methods Which were given tracheal intubation anesthesia and laryngeal mask anesthesia respectively Compared with the safety of anesthesia.Results Comparison of MAP and HR index between the two groups before induction and after pneumoperitoneum showed no significant difference(P>0.05).The MAP and HR fluctuation of group B before,during,5min after the laryngeal mask airway,and before the pneumoperitoneum were all smaller than those of group A,with statistically significant differences(P<0.05).The complication rate of group A was significantly higher than that of group B,and the difference was statistically significant(P<0.05).There was no significant difference in postoperative pain scores between the two groups at different time periods(P>0.05).PAW and PetCO2 indexes of patients in both groups were significantly higher than those in front of pneumoperitoneum for 5min after pneumoperitoneum and 15min after pneumoperitoneum,but there was no statistically significant difference between groups(P>0.05).Conclusion For patients with ectopic pregnancy treated by laparoscopic surgery,laryngeal mask anesthesia has less influence on hemodynamics and higher anesthesia safety,which can reduce the occurrence of complications to a greater extent.
