摘要
目的探究2型糖尿病患者中正常范围内的促甲状腺激素水平与高尿酸血症(HUA)之间的关系。方法收集2017年1月-2019年12月在某院内分泌科治疗的351例年龄在18~45岁之间的甲状腺功能正常的T2DM患者。将受试者分为HUA组(n=115)和非HUA组(n=236)。研究正常范围内的促甲状腺激素水平与HUA的关系。结果与非HUA组相比,HUA组的SUA和促甲状腺激素(TSH)较高,差异有统计学意义(P<0.05),而FT4和FT3差异无统计学意义(P>0.05)。按照SUA四分位数分组,随着SUA梯度的增加、TSH、血清肌酐、内源性肌酐清除率比较,差异有统计学意义(P<0.05),FT3和FT4比较差异无统计学意义(P>0.05)。以HUA为因变量,在校正混杂因素后,TSH是甲状腺功能正常的T2DM患者HUA的独立危险因素(OR=1.462,P=0.045)。调整混杂因子后,TSH分为三分位数,TSH最高三分位数与最低三分位数相比,HUA的OR=1.899。结论正常范围内的高TSH是甲状腺功能正常的2型糖尿病患者HUA的独立危险因素。
Objective To explore the relationship between normal thyroid-stimulating hormone levels and hyperuricemia(HUA)in patients with type 2 diabetes.Methods A collection of 351 patients with T2DM with normal thyroid function between the ages of 18 and 45 who were treated in the endocrinology department of a hospital from January 2017 to December 2019.The subjects were divided into HUA group(n=115)and non-HUA group(n=236).To study the relationship between thyroid-stimulating hormone levels within the normal range and HUA.Results Compared with the non-HUA group,the HUA group had higher SUA and thyroid-stimulating hormone(TSH),the difference was statistically significant(P<0.05),but the difference between FT4 and FT3 was not statistically significant(P>0.05).Grouped according to the SUA quartiles,with the increase of the SUA gradient,there was a statistically significant difference in TSH,serum creatinine,and endogenous creatinine clearance(P<0.05),and there was no statistically significant difference between FT3 and FT4(P>0.05).With HUA as the dependent variable,after adjusting for confounding factors,TSH was an independent risk factor for HUA in T2DM patients with normal thyroid function(OR=1.462,P=0.045).After adjusting the confounding factor,TSH was divided into tertiles.Compared with the lowest tertile of TSH,the OR of HUA was 1.899.Conclusion High TSH in the normal range is an independent risk factor for HUA in type 2 diabetic patients with normal thyroid function.
