摘要
目的:系统评价活血消瘿方治疗结节性甲状腺肿的疗效,为中医药治疗结节性甲状腺肿提供循证医学依据。方法:计算机检索Cochrane临床试验数据库、PubMed数据库、中国生物医学数据库、中国知网、维普、万方数据库,查找与活血消瘿方治疗结节性甲状腺肿相关的随机对照临床试验文献,检索时限自建库至2020年3月。由两位研究者独立提取资料并进行质量评价后,采用RevMan5.3统计软件进行Meta分析。结果:共纳入8篇随机对照试验研究文献,合计763例患者。治疗组在临床疗效(OR=5.1,95%CI(3.68,7.08),P<0.01)、中医证候疗效(OR=8.16,95%CI(4.41,15.09),P<0.01)、减小甲状腺结节最大直径(MD=-0.6,95%CI(-0.7,-0.5),P<0.01),降低中医证候积分(SMD=-1.35,95%CI(-1.57,-1.13),P<0.01)等方面均优于对照组,在调节游离三碘甲状腺原氨酸(SMD=-0.35,95%CI(-0.68,-0.03),P=0.03)、游离甲状腺素(SMD=-0.22,95%CI(-0.48,0.04),P=0.09)、促甲状腺激素(SMD=1.12,95%CI(-0.39,2.63),P=0.15)、减小甲状腺体积(MD=0.25,95%CI(-0.82,1.32),P=0.64)等方面较对照组没有优势。结论:活血消瘿方治疗结节性甲状腺肿与常规西药相比具有一定的优势,但仍需纳入具有大样本随机对照试验研究的高质量文献,以提高Meta分析可靠性和准确性。
Objective:To systematically evaluate the efficacy of Huoxue Xiaoying decoction in the treatment of nodular goiter,and to provide evidence-based medicine for the treatment of nodular goiter with traditional Chinese medicine.Methods:Computer searched the Cochrane clinical trial database,PubMed database,China Biomedical Database,CNKI,Weipu,Wanfang database,searched for the randomized controlled clinical trial literature related to the treatment of nodular goiter with Huoxue Xiaoying Recipe.The search time limit is from The database will be built until March 2020.After two researchers independently extracted the data and evaluated the quality,the RevMan5.3 statistical software was used for meta-analysis.Results:A total of 8 randomized controlled trial research literature were included,with a total of 763 patients.In the treatment group,clinical efficacy(OR=5.1,95%CI(3.68,7.08),P<0.01),TCM syndrome efficacy(OR=8.16,95%CI(4.41,15.09),P<0.01),reduction of thyroid The maximum diameter of the nodule(MD=-0.6,95%CI(-0.7,-0.5),P<0.01),reduce the TCM syndrome score(SMD=-1.35,95%CI(-1.57,-1.13),P<0.01)and other aspects are better than the control group,in regulating free triiodothyronine(SMD=-0.35,95%CI(-0.68,-0.03),P=0.03),free thyroxine(SMD=-0.22,95%CI(-0.48,0.04),P=0.09),thyroid stimulating hormone(SMD=1.12,95%CI(-0.39,2.63),P=0.15),reduced thyroid volume(MD=0.25,95%CI)((-0.82,1.32),P=0.64)and other aspects have no advantage over the control group.Conclusion:Huoxue Xiaoying Recipe has certain advantages compared with conventional western medicine in the treatment of nodular goiter,but high-quality literature with large samples of random controlled trials must be included in order to improve the reliability and accuracy of meta-analysis.
