摘要
目的:研究脑CT灌注成像(CTP)评估症状性颅内动脉狭窄(ICAS)患者脑卒中复发风险的应用价值。方法:选取症状性ICAS患者73例,均经CT血管造影(CTA)筛选出责任血管,连续随访观察1年,比较卒中复发组与未复发组CTP上脑血流量(CBF)、脑血容(CBV)、平均通过时间(MTT)、达峰时间(TTP)参数变化。结果:12例在1年内卒中复发(16.44%,12/73);复发组末次随访时,CBF、CBV低于初发卒中时,MTT、TTP长于初发卒中时;末次随访时复发组CBF、CBV低于未复发组,MTT、TTP长于未复发组(P<0.05);Cox回归分析显示,CBF低、CBV低、MTT长、TTP长为预测症状性ICAS患者卒中复发的危险因素(P<0.05)。结论:症状性ICAS患者CTP灌注成像参数与其卒中复发风险有密切关系,可作为临床预后的客观指标。
Objective:To study the application value of cerebral CT perfusion imaging(CTP)in evaluating the risk of stroke recurrence in patients with symptomatic intracranial artery stenosis.Methods:73 cases of ICAS patients were seletced,and the responsible vessels were screened by CT angiography(CTA).After one year of follow-up,the parameters of upper cerebral blood flow(CBF),cerebral blood volume(CBV),mean passing time(MTT),peak reaching time(TTP)were compared between the stroke recurrence group and the non-recurrence group.Results:There were 12 cases recurred within 1 year(16.44%,12/73).At the last follow-up in recurrence group,the CBF,CBV were lower than those at initial stroke,and the MTT,TTP were longer than those at initial stroke.At the last follow-up,the CBF,CBV of recurrence group were lower than those of non-recurrence group,and the MTT,TTP were longer than those of non-recurrence group(P<0.05).Cox regression analysis showed that low CBF,low CBV,long MTT and long TTP were risk factors for predicting stroke recurrence in symptomatic ICAS patients(P<0.05).Conclusion:CTP perfusion imaging parameters of symptomatic ICAS patients are closely related to their risk of stroke recurrence and can be used as an objective index of clinical prognosis.
