摘要
痤疮的发生与遗传、痤疮丙酸杆菌繁殖、炎症和免疫反应以及内分泌因素如性激素及胰岛素抵抗等密切相关,其中雄激素作用下的皮脂腺快速发育和脂质大量分泌是痤疮发生的病理生理基础。由内分泌而引起的高雄性痤疮主要有多囊卵巢综合征(PCOS)以及与PCOS相关的HAIR-AN综合征、SAHA综合征等,以PCOS最为常见。王玉玺认为,多囊卵巢综合征高雄激素型痤疮的病机关键在于冲任失和,阴阳失衡,治疗上兼顾标本,调摄冲任,顺应月经周期阴阳变化调整用药,同时不忘行气祛邪以通经络之壅滞,疗效显著。
The occurrence of acne is closely related to heredity,propionibacterium acnes reproduction,inflammation and immune response,as well as endocrine factors such as sex hormones and insulin resistance,among which the rapid development of sebaceous glands and massive secretion of lipids under the action of androgens constitute its pathophysiological basis.Hyperandrogenitic acne caused by endocrine factors mainly includes polycystic ovary syndrome(PCOS),HAIR-AN syndrome and SAHA syndrome,with the latter two types related to PCOS and PCOS the most common type.Professor Wang Yuxi believes that the key to the pathogenesis of hyperandrogenitic acne with polycystic ovary syndrome lies in the disharmony of the thoroughfare channel and conception channel,imbalance between yin and yang.He stresses that in the treatment of acne,both its root causes and symptoms should be taken into consideration,the thoroughfare channel and conception channel be regulated,medications be adjusted in accordance with the changes of yin and yang during a menstrual cycle,and promoting qi circulation for eliminating evils and removing stagnation in meridians not be overlooked.The treatment method turns out to have a remarkable curative effect.
