摘要
目的探讨经阴道病灶切除术治疗剖宫产瘢痕妊娠(CSP)的效果及对子宫肌层损伤的影响。方法将80例CSP患者随机分为对照组(40例,经腹病灶切除术)与观察组(40例,经阴道病灶切除术)。比较两组的临床效果。结果两组的治疗总有效率比较,差异无统计学意义(P>0.05)。观察组的手术时间、血β-HCG转阴时间、住院时间、月经来潮时间及术后首次肛门排气时间均短于对照组,术中出血量少于对照组(P<0.05)。观察组的子宫前壁、子宫后壁、宫底壁厚度均大于对照组(P<0.05)。观察组的并发症总发生率低于对照组(P<0.05)。术后12、24 h,观察组的疼痛评分低于对照组(P<0.05)。结论经阴道病灶切除术治疗CSP患者可获得和经腹病灶切除术相当的效果,且其有助于缩短手术时间,加快患者术后恢复速度,减小子宫肌层损伤,降低并发症发生率。
Objective To explore the effect of transvaginal lesion resection in the treatment of cesarean scar pregnancy(CSP)and its influence on myometrial injury.Methods A total of 80 patients with CSP were randomly divided into control group(40 cases,transabdominal lesion resection)and observation group(40 cases,transvaginal lesion resection).The clinical effects of the two groups were compared.Results There was no significant difference in the total effective rate of treatment between the two groups(P>0.05).The operation time,bloodβ-HCG negative time,hospital stay,menstruation time and postoperative first anal exhaust time in the observation group were shorter than those in the control group,intraoperative blood loss was less than that in the control group(P<0.05).The thickness of the anterior wall,posterior wall and fundus of uterus in the observation group were greater than those in the control group(P<0.05).The total incidence of complications in the observation group was lower than that in the control group(P<0.05).At 12 and 24 h after operation,the pain scores in the observation group were lower than those in the control group(P<0.05).Conclusion Transvaginal lesion resection in the treatment of CSP patients can obtain the same effect as transabdominal lesion resection,which is helpful to shorten the operation time,accelerate the recovery speed,and reduce the myometrial injury and the incidence of complications.
