摘要
目的:分析加压交锁髓内钉内固定对股骨转子下长段粉碎性骨折患者术后骨折愈合时间及髋关节功能的影响。方法:选取我院接收的股骨转子下长段粉碎性骨折患者82例,行解剖型锁定加压钢板内固定治疗的40例为钢板组,行加压交锁髓内钉内固定治疗的42例为髓内钉组,对比两组引流液量、切口长度、手术失血量、手术时间、早期下床活动时间、骨折愈合时间、随访6个月髋关节恢复状况。结果:髓内钉组引流液量、手术失血量少于钢板组,切口长度小于钢板组,手术时间、骨折愈合时间短于钢板组(P<0.05);随访6个月,髓内钉组髋关节恢复状况优于钢板组(P<0.05)。结论:加压交锁髓内钉内固定治疗股骨转子下长段粉碎性骨折,可优化手术情况,加快骨折愈合时间,促进髋关节功能恢复。
Objective:To analyze the effects of compression and interlocking intramedullary nail internal fixation in patients with subtrochanteric long-segment comminuted fractures. Methods:A total of 82 patients with subtrochanteric long-segment comminuted fractures in our hospital were selected. 40 patients who received anatomical locking compression plate internal fixation were treated as the plate group,and 42 patients who received compression and interlocking intramedullary nail internal fixation were treated as the intramedullary nail group. The amount of drainage fluid,length of incision,blood loss during operation,operation time,time to get out of bed early,fracture healing time,and hip recovery after 6 months of follow-up were compared between the two groups. Results:The amount of drainage and blood loss in the intramedullary nail group was less than that of the plate group,the incision length was shorter than that of the plate group,and the operation time and fracture healing time were shorter than that of the plate group(P<0.05). After 6 months of follow-up,the recovery of the hip joint in the intramedullary nail group was better than that of the plate group(P<0.05). Conclusion:Compression and interlocking intramedullary nail internal fixation for the treatment of subtrochanteric long-segment comminuted fractures can optimize the operation situation,accelerate the healing time of fractures,and promote the recovery of hip joint function.
