摘要 目的探讨阿尔茨海默病(A1zheimer’s disease,AD)全脑功能连接强度(functional connectivity strength,FCS)、局部脑血流量(cerebral blood flow,CBF)及CBF/FCS的改变以及CBF/FCS改变与神经心理评分之间的相关性。方法26例AD患者和34例正常对照组进行多模态磁共振检查及神经心理学测试。采用双样本t检验进行影像特征之间的比较,采用偏相关分析评估组间差异脑区的CBF/FCS值与神经心理评分的相关性。结果AD患者默认网络相关脑区血流灌注减低,双侧颞中回FCS值显著降低。此外,AD患者左侧颞中回CBF/FCS值减低,并与日常生活能力减退及整体认知水平下降相关。结论AD患者出现显著的血流灌注减低、核心脑区功能减退以及神经血管失偶联,这些改变可能是其潜在病理生理机制的一部分,左侧颞中回的CBF/FCS下降可能成为AD早期诊断的影像学标记物。 Objective To study the changes of whole brain functional connectivity strength(FCS),regional cerebral blood flow(CBF),and CBF/FCS in Alzheimer's disease(AD),as well as the correlations between CBF/FCS alterations and neuropsychological scores.Methods Twenty-six AD patients and 34 normal controls underwent multimodal magnetic resonance imaging examinations and neuropsychological tests.Two-sample T tests were used to compare the image characteristics,and partial correlation analysis was used to evaluate the correlations between the CBF/FCS alterations and the neuropsychological scores.ResultsSome default mode network-related brain regions showed significantly decreased blood perfusion in AD group.The FCS values of the bilateral middle temporal gyri(MTG)of AD patients were significantly reduced.In addition,AD patients showed decreased CBF/FCS in left MTG,and greater decrease was associated with more severe impairment in the daily living ability and overall cognition.Conclusion AD patients show significant blood perfusion reduction,core brain dysfunction,and neurovascular decoupling.These changes may be parts of its potential pathophysiological mechanisms.The CBF/FCS reduction in the left MTG may be an early imaging marker for early diagnosis of AD.
