摘要
目的分析功能连接密度(FCD)和低频振幅(ALFF)等脑功能指标与重复经颅磁刺激(rTMS)治疗抑郁症患者临床疗效的相关性。方法重庆医科大学附属第一医院精神科于2017年3月至2018年3月通过招募纳入符合入组条件的住院或者门诊抑郁症患者27例,进行rTMS治疗10次,通过治疗前、后汉密尔顿抑郁量表(HAMD)评分变化来反映其抗抑郁效应。同时,收集所有患者在rTMS治疗前后的静息态功能磁共振数据,提取基线状态下的FCD和ALFF作为反映静息态下脑功能指标,并将其与rTMS临床疗效相关性应用REST软件进行相关分析。结果左侧中央前回和左侧舌回的ALFF与rTMS的抗抑郁效应呈显著负相关(r=0.55、0.49,P<0.05),左侧腹外侧前额叶与后扣带/楔前叶的ALFF与rTMS的抗抑郁效应呈显著正相关(r=0.77、0.69,P<0.05);右侧梭状回的FCD与rTMS抗抑郁效应呈显著正相关(r=0.5,P<0.05)。结论抑郁症患者基线状态下的左侧腹外侧前额叶和后扣带回/楔前叶的ALFF越高,右侧梭状回的FCD越高,预示rTMS的抗抑郁效应越好。左侧中央前回和左侧舌回的ALFF越低,提示rTMS的抗抑郁效应越好。
Objective To analyze the correlation between resting-state brain function indicators(functional connection density[FCD],amplitude of low frequency fluctuation[ALFF])and the efficacy of repetitive transcranial magnetic stimulation(rTMS)for patients with depression.Methods Twenty-seven patients with depression were recruited from the Department of Psychiatry,the First Affiliated Hospital of Chongqing Medical University from March 2017 to March 2018.All of them were treated with rTMS for 10 times.The antidepressant effect of rTMS was reflected by the changes of Hamilton Depression Scale(HAMD)scores after treatment.At the same time,resting-state fMRI data of each patient was collected at baseline,and then FCD and ALFF were extracted as indicators of resting-state brain function.Correlation analysis was used between HAMD changes and FCD and ALFF at baseline.Results ALFF of left precentral gyrus and left lingual gyrus was significantly negatively correlated with the antidepressant effect of rTMS(correlation coefficients were 0.55 and 0.49,P<0.05),while ALFF of left ventrolateral prefrontal cortex and posterior cingulated/precuneus were significantly positively correlated with rTMS effects(correlation coefficients were 0.77 and 0.69,P<0.05).There was a significant positive correlation between FCD in right fusiform gyrus and rTMS effects(correlation coefficients was 0.5,P<0.05).Conclusion At baseline,the higher the ALFF in left ventrolateral prefrontal cortex and posterior cingulated/precuneus,the higher the FCD in the right fusiform gyrus,which predicts better anti-depressant effects of rTMS.The lower the ALFF of left precentral gyrus and left lingual gyrus,the better the antidepressant effect of rTMS.
