摘要
目的:总结累及皮肤为主的不同类型组织细胞增生症临床病理特征、鉴别诊断及共同点。方法:分析比较4例组织细胞增生症患者临床特征、组织病理学及免疫组化特点并结合文献进行分析。结果:4例患者有3例发生于儿童,年龄在1~16个月之间,1例为老年女性,病程在1~6个月,发病部位分别为会阴、面部、右前胸及前胸,呈肤色、黄红色及红色丘疹、斑块,其它系统检查未发现明显异常,通过病理及免疫组化诊断分别为朗格罕组织细胞增生症、良性头部组织细胞增生症、幼年黄色肉芽肿及皮肤Rosai-Dorfman病。结论:组织细胞增生症临床表现多样,可累及皮肤及其他系统,需要组织病理及免疫组化进行确诊,不同类型有类似的临床和组织学特点,代表疾病谱中不同的阶段,仅累及皮肤者症状较轻,但需要全面检查及随访排除系统病变。
Objective:To summarize the clinicopathological characteristics and differential diagnosis of the different types of histiocytosis mainly involving the skin.Methods:The clinical,histopathological and immunohistochemical characteristics of 4 patients with histiocytosis were analyzed and compared with literature review.Results:Three of four patients were children,aged between 1 month and 16 months.The fourth patient was an elderly woman.The course of the diseases ranged from 1 month to 6 months.The patients presented with skin-colored,yellowish-red,or red papules and plaques.The lesions affected the perineum,face,or chest,respectively.No obvious abnormalities were found by systematic evaluation.The patients were diagnosed as Langerhans cell histiocytosis,benign cephalic histiocytosis,juvenile xanthogranuloma or cutaneous Rosai Dorf-man’s disease,respectively,by histopathologic and immunohistochemical examination.Conclusion:The clinical presentations of histiocytosis vary greatly,may involving skin and other organ systems.The diagnosis of histiocytosis requires histopathologic and immunohistochemical examination.Different types of histiocytosis may have similar clinical and histological characteristics,representing different stages of the spectrum of disease.If only the skin is affected,the symptoms are mild.However,a com-prehensive examination and follow-up are necessary to exclude systemic diseases.
