摘要
【目的】观察宽筋熏洗汤联合塞来昔布胶囊治疗风寒湿阻型膝骨关节炎性滑膜炎的临床疗效。【方法】将60例风寒湿阻型膝骨关节炎性滑膜炎患者随机分为治疗组和对照组,每组各30例。2组患者均给予塞来昔布胶囊口服治疗,在此基础上,治疗组给予宽筋熏洗汤熏洗治疗,对照组给予双氯芬酸二乙胺乳胶剂外涂治疗,疗程为2周。观察2组患者治疗前后疼痛视觉模拟量表(VAS)评分、膝关节周径、Lysholm膝关节功能评分及血中超敏C反应蛋白(hs-CRP)水平的变化情况,并评价2组患者的临床疗效。【结果】(1)治疗2周后,治疗组的总有效率为93.3%(28/30),对照组为73.3%(22/30),组间比较(秩和检验),治疗组的疗效明显优于对照组,差异有统计学意义(P<0.05)。(2)治疗后,2组患者的VAS评分、膝关节周径、Lysholm膝关节功能评分及血中hs-CRP水平均较治疗前明显改善(P<0.05),且治疗组的改善作用均明显优于对照组,差异均有统计学意义(P<0.05)。【结论】宽筋熏洗汤联合塞来昔布胶囊治疗风寒湿阻型膝骨关节炎性滑膜炎疗效显著,可有效缓解患膝疼痛、减轻肿胀、改善关节活动功能及降低血中超敏C反应蛋白水平。
Objective To observe the clinical effect of Kuanjin Decoction for Fumigation and Washing combined with Celecoxib Capsules for the treatment of knee osteoarthritis(KOA)associated synovitis differentiated as the syndrome of wind,cold and dampness blockage.Methods Sixty patients with KOA associated synovitis differentiated as the syndrome of wind,cold and dampness blockage were randomly divided into treatment group and control group,and each group had 30 cases.Both groups were treated with Celecoxib Capsules,and additionally the treatment group was given fumigating and washing with Kuanjin Decoction for Fumigation and Washing,and the control group was treated with external application of Diclofenac Diethylamine Emulgel.The course of treatment for the two groups lasted for 2 weeks.Before and after treatment,the pain scores of visual analogue scale(VAS),knee circumference,Lysholm knee function scores and blood hypersensitive C-reactive protein(hs-CRP)were compared between the two groups.After treatment,the clinical efficacy of the two groups was evaluated.Results(1)After 2 weeks of treatment,the total effective rate in the treatment group was 93.3%(28/30),and that in the control group was 73.3%(22/30).The intergroup comparison with rank sum test showed that the clinical efficacy of the treatment group was superior to that in the control group,and the difference was statistically significant(P<0.05).(2)After treatment,the mean VAS pain scores,knee circumference,Lysholm knee function scores and blood hs-CRP level of the patients in the two groups were much improved in comparison with those before treatment(P<0.05),and the improvement in the treatment group was superior to that in the control group(P<0.05).Conclusion Kuanjin Decoction for Fumigation and Washing combined with Celecoxib Capsules exerts certain effect for the treatment of KOA associated synovitis differentiated as the syndrome of wind,cold and dampness blockage,and the combined therapy is effective on relieving knee pain and swelling,improving joint movemen
