摘要
目的分析平顶山市近十年手足口病病原学构成情况,为科学开展手足口病防控提供数据支持。方法用实时荧光PCR检测手足口病标本,按照Cox A16、EV 71和肠道病毒通用型等类型进行分型。结果平顶山市2009-2018年各年病原构成比差异有统计学意义(χ^(2)=884.88,P<0.01),优势病原由EV 71逐步转变为其他型肠道病毒,各年EV 71比例与重症率呈正相关;流行高峰期,各月的优势病原基本与当年优势病原保持一致;城区的EV 71比例低于郊县;不同性别、年龄和人群类别间病原构成差异均有统计学意义(χ^(2)=6.22,P=0.045;χ^(2)=28.12,P=0.002;χ^(2)=30.06,P<0.01);重症和轻症病例病原构成差异有统计学意义(χ^(2)=527.95,P<0.01)。结论应加强Cox A6、Cox A10等其他型肠道病毒监测,以有效防控手足口病。
Objective To analyze the etiological composition features of hand-foot-mouth disease(HFMD)in Pingdingshan in the last decade to provide data suport for prevention and control of HFMD.Methods The specimens of hand-foot-mouth disease were detected by real-time fluorescence PCR,and were classified according to Cox A16,EV 71 and enterovirus type.Results In Pingdingshan,the differences in pathogenic composition ratio in years thorough from 2009 to 2018(χ^(2)=884.88,P<0.01).The dominant pathogens gradually turn into from EV 71 to other enteroviruses.EV 71 proportion in each year was positively correlated with the severe rate.During the peak epidemic period,the dominant pathogens in each month were basically consistent with the dominant pathogens in that year.The ratio of EV 71 in urban areas was lower than that in suburban counties.The differences in pathogenic compositions of different genders,ages,populations and categories were statistically significant(χ^(2)=6.22,P=0.045;χ^(2)=28.12,P=0.002;χ^(2)=30.06,P<0.01);and the difference in the pathogenic composition between severe and mild cases was statistically significant(χ^(2)=527.95,P<0.01).Conclusion The surveillance on Cox A6,Cox A10 and other enteroviruses should be tightened to effectively prevent and control HFMD.
