摘要
目的探讨子宫内膜异位症(EMs)治疗过程中腹腔镜手术与促性腺激素释放激素激动剂(GnRH-a)联合应用的临床疗效。方法选取60例行腹腔镜手术治疗的EMs患者,根据随机数字表法分为对照组与研究组,每组30例。研究组给予GnRH-a治疗,对照组给予米非司酮治疗。比较两组患者临床疗效、治疗前、治疗后3个月血清性激素水平及治疗后1年复发率及宫内妊娠率。结果研究组患者总有效率为93.33%,明显高于对照组的76.67%,差异有统计学意义(P<0.05)。治疗前,两组患者雌二醇、卵泡刺激素、黄体生成素水平比较,差异无统计学意义(P>0.05);治疗后3个月,两组雌二醇、卵泡刺激素、黄体生成素水平均明显低于治疗前,且研究组明显低于对照组,差异有统计学意义(P<0.05)。研究组患者复发率为3.33%,明显低于对照组的20.00%,宫内妊娠率为36.67%,明显高于对照组的13.33%,差异有统计学意义(P<0.05)。结论腹腔镜手术联合GnRH-a治疗EMs疗效确切,有助于进一步调节患者性激素水平、预防复发并提高宫内妊娠率,值得临床推广与应用。
Objective To investigate the clinical efficacy of laparoscopic surgery combined with gonadotropin releasing hormone agonist(GnRH-A)in the treatment of endometriosis(EMS).Methods 60 EMS patients undergoing laparoscopic surgery were divided into control group and study group according to random number table,with 30 cases in each group.The study group was treated with GnRH-a and the control group with mifepristone.Clinical efficacy,serum sex hormone levels before and 3 months after treatment,recurrence rate and intrauterine pregnancy rate 1 year after treatment were compared between the two groups.Results The total effective rate of the study group was 93.33%,which was significantly higher than that of the control group(76.67%),the diff erence was statistically signifi cant(P<0.05).Before treatment,there was no statistical signifi cance in the levels of estradiol,follicle stimulating hormone and luteinizing hormone between 2 groups(P>0.05).3 months after treatment,the levels of estradiol,follicle stimulating hormone and luteinizing hormone in both groups were signifi cantly lower than before treatment,and those in the study group were signifi cantly lower than those in the control group,with statistical signifi cance(P<0.05).The recurrence rate of the study group was 3.33%,which was significantly lower than that of the control group(20.00%),and the intrauterine pregnancy rate was 36.67%,which was significantly higher than that of the control group(13.33%),with statistical signifi cance(P<0.05).Conclusion Laparoscopic surgery combined with GnRH-a has a defi nite eff ect on EMS,which is helpful to further regulate the sex hormone level of patients,prevent recurrence and improve intrauterine pregnancy rate,and is worthy of clinical promotion and application.
