摘要
目的探讨玻璃体切除术后并发性白内障患者在无玻璃体腔灌注情况下行白内障超声乳化手术的安全性。方法玻璃体切除术后并发性白内障患者32例,术前排除晶状体悬韧带及后囊膜损伤,在无玻璃体腔灌注下实施低灌注、低负压超声乳化手术。结果32例均顺利完成白内障超声乳化手术。术中无后囊膜破裂,亦无悬韧带损伤和眼内出血;4例后囊膜混浊抛光不净。术后视力均有不同程度提高。术后第1天角膜切口水肿9例,术后1周消失。术后1个月时,4例后囊膜混浊病例行YAG后囊激光切开术,视力提高。结论对玻璃体切除术后的白内障患者,若无晶状体悬韧带及后囊膜损伤,不行玻璃体腔灌注也能完成白内障手术,可减少创伤和眼内操作。
Objective To explore the safety of cataract phacoemulsification without vitreous cavity perfusion in the patients with cataract after vitrectomy.Methods The low perfusion and negative pressure phacoemulsification without vitreous cavity perfusion was performed in 32 patients with complicated cataract after vitrectomy,in whom the injury of the lens suspension ligament and posterior capsule was excluded.Results Cataract phacoemulsification was performed successfully in all the 32 patients without posterior capsular rupture,injury of suspended ligament and intraocular hemorrhage.There were four cases with turbid and polished posterior capsule.The visual acuity was improved in different degrees after operation.Corneal incision edema was observed in 9 cases on the 1st day after operation,which disappeared after one week.The YAG posterior capsular laser resection was performed in 4 cases one month after operation with improved visual acuity.Conclusion The cataract phacoemulsification without vitreous cavity perfusion is safe and reliable with reduced trauma and intraocular operations as long as there is no injury of the lens suspension ligament and posterior capsule in the patients with cataract after vitrectomy.
