摘要
目的探究慢性阻塞性肺疾病合并呼吸机相关性肺炎患者的病原菌分布及耐药性。方法收集2016年3月—2019年9月三亚中心医院收治的140例慢性阻塞性肺疾病合并呼吸机相关性肺炎患者的痰液标本,分离鉴定致病菌及其抗生素敏感性。结果从140份样本中检出210株病原菌,其中革兰阴性菌138株,占65.71%;革兰阳性菌49株,占23.33%;真菌23株,占10.95%。肺炎克雷伯菌对美罗培南、亚胺培南的耐药率为0,铜绿假单胞菌和鲍曼不动杆菌的耐药率低于10%;金黄色葡萄球菌、溶血葡萄球菌对氨苄西林、红霉素耐药率较高,对万古霉素、利奈唑胺耐药性较低。结论慢性阻塞性肺疾病合并呼吸机相关性肺炎的病原菌构成较复杂,以革兰阴性菌为主,对常用抗生素耐药,应按照药敏结果针对性用药。
Objective To explore the distribution and drug resistance of pathogenic bacteria in patients with chronic obstructive pulmonary disease(COPD)and ventilator-associated pneumonia(VAP).Methods A total of 140 patients with COPD and VAP who were admitted to Sanya Central Hospital from March 2016 to September 2019 were en-rolled in the study.The sputum samples were collected,pathogenic bacteria were isolated and identified,and their antibiotic susceptibility was tested.Results A total of 210 pathogenic bacteria strains were detected from 140 samples,of which 138(65.71%)were Gram-negative bacteria,49(23.33%)were Gram-positive bacteria,and 23 strains(10.95%)were fungi.The resistance rate of Klebsiella pneumoniae to meropenem and imipenem was 0,and the resistance rates of Pseudomonas aeruginosa and Acinetobacter baumannii were less than 10%.Staphylococcus aureus and S.haemolyticus had higher resistance rates to ampicillin and erythromycin,but lower resistance to vancomycin and linezolid.Conclusion The composition of pathogenic bacteria,was complicated mainly Gram-negative bacteria in patients with COPD and VAP,which were resistant to commonly used antibiotics,and medications should be tailored according to the drug susceptibility results.
