摘要
目的:探讨髂静脉压迫(IVC)在无症状人群中的发生率及其临床特征。方法:回顾性分析2018年8月至2019年8月香港大学深圳医院收治的无血管疾病而行全腹增强CT检查的患者的影像学及临床资料,于不同水平面测量髂静脉前后径,并根据公式计算出髂静脉受压程度。以受压程度50%为界分狭窄组与非狭窄组,分析比较两组在性别、年龄、体重指数(BMI)和凝血功能方面的差异。结果:共有692例患者纳入该研究,男性351例(50.7%),年龄(38.76±10.90)岁(范围:18~71岁),BMI(22.79±3.72)kg/m^(2)。IVC总体受压百分比中位数为43(19,65)%,不同受压程度所占的比例分别为:<25%者74例(10.7%),≥25%且<50%者368例(53.2%),≥50%且<75%者196例(28.3%),≥75%者54例(7.8%)。狭窄组与非狭窄组相比,性别、年龄、BMI、凝血酶时间及活化部分凝血酶时间在两组间有统计学差异(P均<0.05)。结论:髂静脉受压程度≥50%者在无症状人群中较为多见。男性、高龄、肥胖可能与髂静脉受压的发生相关。
Objective To investigate the incidence and clinical characteristics of iliac vein compression(IVC)in asymptomatic patients.Methods The radiological and clinical data of patients with non-vascular disease who underwent whole abdominal contrast-enhanced CT scan in the University of Hong Kong-Shenzhen Hospital from August 2018 to August 2019 were retrospectively analyzed.The anteroposterior diameter of iliac vein was measured from different horizontal planes,and then the compression degree was calculated according to the formula.A compression degree of≥50%was defined as iliac vein stenosis.Comparison of variables including gender,age,body mass index(BMI)and coagulation function between the stenotic and non-stenotic group was performed.Result A total of 692 patients were enrolled in this study including 351 males(50.7%)with a mean age of(38.76±10.90)years(range,18-71 years),and mean BMI of(22.79±3.72)kg/m2.The median of IVC rate was 43(21.0,66.0)%,and the proportions of different IVC degrees were as follows:74 cases(10.7%)had<25%compression,368 cases(53.2%)had≥25%and<50%compression,196 cases(28.3%)had≥50%and<75%compression,and 54 cases(7.8%)had≥75%compression.There were significant differences between the two groups in terms of gender,age,BMI,prothrombin time and activated partial thromboplastin time.Conclusion Patients with a degree of IVC≥50%are not rare.The incidence of IVC may be related to male gander,advanced age and obesity.
