摘要
胃癌是肿瘤科常见恶性肿瘤之一,在消化道恶性肿瘤的发病中高居首位,疼痛是胃癌患者最常见的症状之一,胃癌本身和各种抗肿瘤治疗措施均可导致疼痛的发生。传统止痛治疗主要以WHO三阶梯给药为主,但阿片类药物具有较大的不良反应及成瘾性,导致部分患者不能耐受及依从性降低,以至不能满足患者的止痛要求,影响患者的生活质量。查阅近年来针灸治疗胃癌相关性疼痛的高质量研究文献,发现针灸具有止痛效果广泛、不良反应少、无成瘾性的特点,且对患者有一定免疫调节及减少药物不良反应的作用,在胃癌患者的疼痛治疗中发挥重要作用,但高质量的临床研究数量较少,不足以形成高质量的循证医学证据来证明针灸治疗疼痛的确切疗效。胃癌患者的疼痛主要分为两类,一是胃癌术后因手术创伤而产生的疼痛,二是肿瘤自身所引起的癌性疼痛,近年国内外相关临床研究主要以普通针刺、电针、耳穴压豆、揿针、穴位注射、艾灸及火针等疗法为主,故文从胃癌术后疼痛和胃癌癌痛两方面来探讨不同的针灸疗法治疗本病的疗效,并对本方面的研究进行述评与讨论,为今后的相关研究提供参考。随着临床研究设计的不断进步,多中心大样本实验的完善,研究结果将更具有说服力,从而更好地指导胃癌患者疼痛的针灸治疗。
Gastric cancer is one of the most common malignant tumors in oncology department.It ranks the first place in the incidence of gastrointestinal malignancise and pain is one of the most common symptoms of gastric cancer patients.Gastric cancerand various anti-tumor treatment measures can lead to pain.Although the traditional analgesic treatment is mainly WHO three-step administration,opioid drugs have a large adverse reaction and addiction,resulting in any patients can not tolerate and decreased compliance,and so it can not meet the pain relief requirements of patients,affecting the quality of life.Referring to the high-quality research literature of acupuncture and the treatment of gastric cancer related pain in recent years,it is found that acupuncture has the characteristics of extensive analgesic effect,few side effects and no addiction,and has immunomodulation effect on patients and reduces adverse drug reactions,and it plays an important role in the pain treatment of gastric cancer patients.But the number of high-quality clinical studies is small,which is not enough to form high-quality evidence-based medical evidence of acupuncture in the treatment of pain.The pain of gastric cancer patients is divided into two main categories,one is the pain caused by surgical trauma and the other is the cancer pain caused by tumor itself.In recent years,the main clinical studies at home and abroad are about common acupuncture,electroacupuncture,auricular point acupressure,thumb-tack needle,acupoint injection,moxibustion and fire needle.Therefore,this article,from the gastric cancer postoperative pain and gastric cancer pain two aspects,explored the efficacy of different acupuncture therapies for this disease,and reviewed and discussed the research in this area,and provided reference for future related research.With the continuous progress of clinical research design and the improved multi-center large sample experiment,the results of the research will be more convincing,and thus better guiding the acupuncture treatment of
