摘要
目的评估鼻内镜下泪囊鼻腔吻合术(E-DCR)中吻合口应用药物支架的效果。方法前瞻性研究。以解放军第988医院2017年12月至2019年6月慢性泪囊炎行E-DCR 50例(60眼)为研究对象。患者随机分为两组:治疗组,25例(31眼),E-DCR的吻合口处放置药物支架;对照组,25例29眼,术中不放置药物支架。术后随访1年,进行观察比较。结果治疗组治愈28眼(90.32%),好转2眼,复发1眼;对照组治愈14眼(48.28%),好转8眼,复发7眼(χ^(2)=4.000,P=0.047)。治疗组术中出血(35±15)mL,少于对照组的(40±18)mL(t=2.152,P=0.035)。治疗组术后吻合口闭合者1眼,少于对照组的7眼。治疗组术后鼻部无需再用糖皮质激素类鼻喷雾剂,无疼痛感;而对照组术后8眼出现疼痛感。结论吻合口药物支架在E-DCR术中应用,效果良好。
Objective To evaluate the efficacy of anastomotic drug stent applied in endoscopic dacryocystorhinostomy(E-DCR).Methods This was a prospective study.A total of 60 eyes of 50 patients with chronic dacryocystitis who underwent E-DCR from Dec.2017 to Jun.2019 in the 988 Hospital of PLA were selected as the research objects.These patients were randomly divided into two groups.Total of 31 eyes of 25 patients received an anastomotic drug stent placed in the anastomosis site of dacryocystorhinostomy were in the treatment group.Total of 29 eyes of 25 cases without the anastomotic drug stent during the operation were in the control group.The follow-up time was 1 year after the operation.The efficacy were observed and compared between the two groups.Results In the treatment group,28 eyes(90.32%)were cured,2 eyes were improved,and 1 eye relapsed.In the control group,14 eyes(48.28%)were cured,8 eyes were improved,and 7 eyes were ineffective.The effective rate in the treatment group was better than that in the control group(χ^(2)=4.000,P=0.047).The intraoperative blood loss were(35±15)mL in the treatment group,which was less than that of(40±18)mL in the control group(t=2.152,P=0.035).In the treatment group,anastomotic stoma of 1 eye was closed after operation,which was less than 7 eyes in the control group.There was no need to use glucocorticord nasal spray in the nasal cavity after surgery and patients felt no pain in the treatment group.However,8 eyes of patients felt pained after surgery in the control group.Conclusion The application of anastomotic drug stent is effective for the treatment of endoscopic dacryocystorhinostomy(E-DCR).
