摘要
目的:调查成人恶性胶质瘤住院病人术前营养状况,分析病人营养状况与临床预后的相关性。方法:采用描述性研究,对2018年12月至2019年12月在北京天坛医院神经外科病房进行治疗的184例胶质瘤病人进行调查,应用NRS 2002对其进行营养风险筛查,分析术前病人的营养状况与术后入住ICU时间、机械通气时间、住院时间、并发症、住院费用等的相关性。结果:184例胶质瘤病人营养风险发生率为43.48%(80/184),有营养风险的病人其术后入住ICU的天数、机械通气天数、总住院天数、肺部感染的发生率、住院费用均高于无营养风险的病人,差异有显著性统计学意义(P<0.05)。结论:胶质瘤病人的营养风险发生率较高,术前存在营养风险的病人临床预后显著差于无营养风险的病人。临床医护人员应重视胶质瘤病人的营养状况,将术前营养状态评估列为必要的评估内容,及早帮助有营养风险的病人进行营养支持,改善其临床预后,提高病人的生存质量。
Objective:To investigate the preoperative nutritional status of adult patients with malignant glioma and analyze the correlation between nutritional status and clinical prognosis.Methods:Using descriptive research,between December 2018 and December 2019,184 patients with glioma were treated in Department of Neurosurgery,Beijing Tiantan hospital,and nutritional risk screening was applied using NRS 2002.The nutritional status of patients with preoperative was recorded and correlated with ICU admission time,mechanical ventilation days,length of hospital stay and complications.Results:The incidence of nutritional risk was 43.48%(80/184).The number of days of ICU admission,mechanical ventilation,total hospital stay,pulmonary infection and hospitalization expenses in patients with nutritional risk were all higher than those without nutritional risk(P<0.05).Conclusion:The incidence of nutritional risk in glioma patients is high,which has a great impact on postoperative clinical prognosis.Preoperative nutritional status evaluation should be included as a necessary evaluation content,and nutritional support should be provided to patients with nutritional risk as soon as possible to improve their clinical prognosis and quality of life.
