双能量CT冠脉、心肌成像辐射剂量的优化研究 被引量:2

Dual energy coronary computed tomography angiography with low radiation dose-A feasibility study

摘要 目的通过对双能量冠状动脉CT造影(DE-CCTA)不同扫描方式及重建方式下冠脉、心肌成像质量的综合评价及对比分析,探讨DE-CCTA辐射剂量优化的可行性。方法连续收集2016年2月至2016年8月于新疆医科大学第一附属医院影像中心因体检或疑似冠心病行DE-CCTA检查的68例患者资料,纳入其中40例并随机分至常规剂量组及降剂量组,常规剂量组采取常规扫描模式,并对图像进行滤波反投影重建,降剂量组采用低剂量扫描模式,并对图像进行迭代算法重建;对两组成像的辐射剂量进行比较,测量并比较两组主动脉根部以及左心室外侧壁心肌的图像噪声、信噪比、对比噪声比,以及前降支及左心室心肌成像质量的主观评分。结果降剂量组有效辐射剂量(4.68±0.86)mSv较常规剂量组有效辐射剂量(6.69±0.85)mSv显著降低,差异有统计学意义(P<0.01);降剂量组较常规剂量组主动脉对比噪声比升高(54.79±10.35 vs 43.66±10.79)、心肌对比噪声比升高(20.58±4.04 vs 16.93±3.60),差异有统计学意义(P值均<0.05),主动脉及心肌的噪声、信号噪声比、主观评分在两组间差异无统计学意义(P值均>0.05)。结论低剂量DE-CCTA扫描模式结合迭代重建能够在保证冠脉及心肌图像质量的前提下有效降低扫描辐射剂量,因此在临床实际操作过程中可根据临床需求适当开展DE-CCTA检查。 Objective To explore feasibility of dual energy coronary computed tomography angiography(DE-CCTA)with low radiation dose.Methods 40 of 68 patients undergoing DE-CCTA were enrolled in this study prospectively and equally assigned to control group and experimental group randomly.The controlgroup(Group A,n=20)were scanned using the conventional tube output and reconstructed using filtered back projection(FBP).The experimental group(Group B,n=20)were scanned using the low radiation dose tube outputand reconstructed using sinogram affirmed iterative reconstruction(SAFIRE).Noise,signal to noise ratio(SNR)and contrast to noise ratio(CNR)of aortic root and myocardium,subjective scoring of left anterior descending branch(LAD)and myocardium,and radiation dose were measured and comparied between the two groups.Results Radiation dose of experimental group(4.68±0.86)mSv was significantly lower than that of control group(6.69±0.85)mSv(P<0.05);Aortic and myocardial CNR were higher in experimental group(54.79±10.35,20.58±4.04)than those in control group(43.66±10.79,16.93±3.60)(all P<0.05);Aortic and myocardial noise,SNR and subjective scoring were not statistically different between the two groups(all P>0.05).Conclutions By combined using of low tube current,automatic current adjustment,minidose and iterative reconstruction (IR) technique, DE-CCTA could reduce radiation dose effectively with precondition ofensuring image quality of coronary artery and myocardium.
