摘要
目的:对比分析带袢钢板弹性固定与螺钉内固定治疗下胫腓联合损伤患者的效果。方法:选取2018年1月至2020年2月收治的60例下胫腓联合损伤患者分为A组和B组,A组给予带袢钢板弹性固定治疗,B组给予螺钉内固定治疗,比较两组手术指标、术后恢复指标、影像学指标、临床疗效及并发症发生率。结果:两组手术时间、术中出血量及切口长度差异无统计学意义(P>0.05),术前两组下胫腓间隙(TBCS)、下胫腓重合(TBOL)差异无统计学意义(P>0.05),术后两组TBCS、TBOL均下降,且B组TBCS、TBOL下降更显著(P<0.05);术后6个月A组临床疗效优良率为83.33%,高于B组的66.67%(P<0.05),术后12个月两组临床疗效优良率差异无统计学意义(P>0.05);A组部分负重下地时间较B组缩短(P<0.05),两组完全负重下地时间差异无统计学意义(P>0.05);两组并发症发生率差异无统计学意义(P>0.05)。结论:与螺钉内固定相比,带袢钢板弹性固定治疗下胫腓联合损伤可及早进行负重训练,保留关节的微动性,提高临床疗效,并且避免了二次手术造成的创伤,值得推广。
Objective:To compare and analyze the effects of elastic fixation with loop steel plates and internal fixation with screws in treating patients with inferior tibiofibular syndesmosis injury.Methods:60 patients with inferior tibiofibular syndesmosis injury admitted from January 2018 to February 2020 were chosen and divided into Group A and Group B.Group A were given elastic fixation with loop steel plates,while Group B were given internal fixation with screws,with the operation indexes,postoperative recovery indexes,imaging indexes,clinical efficacy and complication rates of the two groups compared.Results:There was no significant difference in operation time,intraoperative blood loss and incision length between the two groups(P>0.05).There was no significant difference in TBCS and TBOL between the two groups before operation(P>0.05).After operation,TBCS and TBOL in both groups decreased significantly,and they decreased more significantly in Group B(P<0.05).The excellent and good rates of clinical curative effects in group A was 83.33%6 months after operation,higher than that in Group B(66.67%)(P<0.05).There was no difference in the excellent and good rates of clinical curative effect between the two groups 12 months after operation(P>0.05).The time to ground under partial load in group A was significantly shorter than that in group B(P<0.05),but there was no significant difference in time to ground under full load between the two groups(P>0.05).There was no significant difference in the incidence of complications between the two groups(P>0.05).Conclusion:Compared with internal fixation with screws,elastic internal fixation with loop steel plates can make patients with tibiofibular syndesmosis injury perform weight-bearing training as soon as possible,which preserves the micro-movement of the joint,improves clinical efficacy,and avoid trauma caused by reoperation,worthy of promotion.
