摘要
目的:甲基苯丙胺吸食成瘾会严重影响成瘾者大脑功能,抗阻训练可以提高成瘾者脑健康水平,但影响机制尚需探讨。方法:选取山东省鲁中强制隔离戒毒所男性甲基苯丙胺成瘾者26人,利用便携式fNRIs采集受试者左侧前额叶(LPFC)、右侧前额叶(RPFC)、左侧运动皮层(LMC)和右侧运动皮层(RMC)的氧合血红蛋白浓度信号,计算其4个频率区间的小波相位相干值(WPCO),分析一次中等强度抗阻训练对受试者脑功能连接即时效应和12周连续抗阻训练对受试者脑功能连接累积效应。通过测量问卷测量睡眠、焦虑、抑郁和渴求的状况及与脑区功能性连接的相关性。结果:一次抗阻训练RPFC和LMC的氧合血红蛋白相对浓度显著升高,特别是在肌源性活动和神经源性活动表现更为显著。12周抗阻训练显著改善睡眠、焦虑、渴求水平,且睡眠、焦虑等指标与各脑区功能连接相关的心率活动的WPCO显著负相关,与LPFC-LMC、LPFC-RPFC、LPFC-RMC、RPFC-LMC的呼吸活动的WPCO值均具有负性相关性。12周中等强度抗阻训练静息态测试中,第I频段,训练后受试者LPFC-LMC、LPFC-RPFC、LPFC-RMC、RPFC-LMC、RPFC-RMC的WPCO值显著高于训练前;第Ⅱ频段,训练后受试者LPFC-LMC、LPFC-RPFC、LPFC-RMC、RPFC-LMC、RPFC-RMC的WPCO值显著高于训练前;第Ⅲ频段,训练后受试者各脑区的WPCO值与训练前无显著差异;第Ⅳ频段,训练后受试者LPFC-LMC的WPCO值显著低于训练前。结论:中等强度抗阻训练可以显著激活甲基苯丙胺成瘾者左、右前额叶,增强不同脑区之间连接,尤其加强了顶叶运动区的肌源性活动和神经源性活动;促使前额叶、顶叶充分参与运动控制,改善成瘾者静息状态下前额叶、运动区和枕叶在心率活动、呼吸活动和肌源性活动的联系,减少成瘾者脑区偏侧使用,加强区域同步性,改善睡眠质量、焦虑、抑郁和渴求水平。
Objective:To explore the effects and mechanisms of resistance training on brain health of methamphetamine addicts.Methods:26 male methamphetamine addicts from the Compulsory Isolation and Rehabilitation Center of Shandong Province were recruited in this study,and the portable fNRIs was used to analyze the immediate effect of a moderate intensity resistance training program and the cumulative effect of 12 weeks continuous resistance training on brain functional connectivity.Results:After one bout of resistance training,the relative concentration of oxyhemoglobin in RPFC and LMC were increased significantly,especially in myogenic activity and neurogenic activity.The 12 weeks of resistance training significantly improved sleep quality and anxiety,which were negatively correlated with the WPCO of heart rate activity related to functional connectivity of brain regions,and negatively correlated with the WPCO of respiratory activity of LPFC-LMC,LPFC-RPFC,LPFC-RMC and RPFC-LMC.In the resting state test of 12-week moderate intensity resistance training,the WPCO values of LPFC-LMC,LPFC-RPFC,LPFC-RMC,RPFC-LMC and RPFC-RMC were significantly higher than that of pre-training in band I and II;in band III,there was no significant difference in the WPCO values of each brain area compared with pre-training;in bandⅣ,the WPCO value of LPFC-LMC was significantly lower than that of pre-training.Conclusions:Moderate intensity resistance training can significantly activate the left and right prefrontal lobes of methamphetamine addicts,and enhance the connections between different brain regions,especially strengthen the myogenic and neurogenic activities in the motor region of the parietal lobe.Moreover,it can promote the prefrontal and parietal lobes to fully participate in motor control,and improve the relationship between heart rate activity,respiratory activity and myogenic activity in the prefrontal,motor and occipital lobes under resting state,and then reduce the side use of the brain area and strengthen regional synchronization
