摘要
目的探讨阴道镜检查在子宫颈上皮内瘤变患者中的应用效果及效能ROC曲线结果。方法选择2018年3月—2020年1月宫颈筛查异常患者126例为对象,所有患者均行阴道镜检查,考虑鳞状上皮病变,包括低级别鳞状上皮内病变(low grade squamous intraepithelial lesions,LSIL)及高级别鳞状上皮内病变(high grade squamous intraepithelial lesions,HSIL),经阴道镜指引下宫颈活检组织病理学检查确诊(“金标准”),并将阴道镜检查结果与“金标准”进行比较;绘制ROC曲线,分析阴道镜检查在子宫颈上皮内瘤变患者中的诊断效能(灵敏度、特异度)。结果126例入组的子宫颈上皮内瘤变患者均经病理组织检查确诊,患者中CINⅠ级64例,CINⅡ级34例,CINⅢ级20例,浸润癌8例。患者均完成阴道镜检查,最终确诊105例,诊断符合率为83.33%;ROC曲线结果表明:阴道镜检查用于子宫颈上皮内瘤变患者中AUC值为0.823,诊断灵敏度为84.50%,诊断特异度为68.10%。结论阴道镜检查用于子宫颈上皮内病变患者中能获得较高的诊断符合率,临床诊断灵敏度较高,可为患者诊疗提供参考依据。
Objective To explore the application effect and ROC curve of colposcopy in patients with cervical intraepithelial neoplasia.Methods 126 patients with abnormal cervical screening from March 2018 to January 2020 were selected as subjects.All patients were examined by colposcopy,considering squamous epithelial lesions,including LSIL and HSIL,which were confirmed by cervical biopsy histopathology under the guidance of colposcopy(gold standard),and the results of colposcopy were compared with the gold standard.ROC curve was drawn to analyze the diagnostic efficiency(sensitivity and specificity)of colposcopy in patients with cervical intraepithelial neoplasia.Results All 126 patients with cervical intraepithelial neoplasia were diagnosed by pathological examination,including 64 cases of CINⅠ,34 cases of CINⅡ,20 cases of CINⅢand 8 cases of invasive cancer.All patients completed colposcopy examination,and 105 cases were finally diagnosed,and the diagnostic coincidence rate was 83.33%.ROC curve showed that the AUC value,diagnostic sensitivity and diagnostic specificity of colposcopy in patients with cervical intraepithelial neoplasia were 0.823,84.50%and 68.10%.Conclusion Colposcopy can obtain high diagnostic coincidence rate and high clinical diagnostic sensitivity in patients with cervical intraepithelial lesions,which can provide reference for patients’diagnosis and treatment.
