摘要
目的研究MRI 3D-TOF结合三维重建技术在原发性三叉神经痛微血管减压术前评估诊断中的应用价值。方法对40例原发性三叉神经痛患者采取MRI 3D-TOF序列扫描,并对三叉神经和其责任血管进行三维重建,以三叉神经与其责任血管的压迫分型为标准,采用以上两种方式分别进行诊断,再由两名权威的影像学主任医师和两名神经外科主任医师对两种诊断方法得到的结果与实际观测得到的诊断结果进行对比,分析神经血管组织的解剖关系。结果术中实际观察:1型16例,2型9例,3型15例;单独使用MRI 3D-TOF检查:1型11例,2型21例,3型8例;MRI 3D-TOF结合三维重建检查:1型11例,2型21例,3型8例;单独使用MRI 3D-TOF检查对患者的诊断结果与术中观察得到的结果,差异具有统计意义(P<0.05);而MRI 3D-TOF结合三维重建得到的诊断结果与术中观察得到的结果没有明显差异(P>0.05)。结论微血管减压前将三维重建技术与3D-TOF序列相结合,能够对三叉神经痛患者病变形态、解剖关系进行全面的评价,对临床诊断和治疗有指导意义,三维重建技术对原发性三叉神经疼痛患者的诊断有重大意义,值得进一步研究和应用。
Objective To study the application value of MRI 3 D-TOF combined with 3 D reconstruction technique in preoperative evaluation and diagnosis of microvascular decompression for primary trigeminal neuralgia.Methods 40 patients with primary trigeminal neuralgia were scanned by MRI 3 D-TOF sequence,and the trigeminal nerve and its responsible vessels were reconstructed in three dimensions.The classification of trigeminal nerve and its responsible vessels was taken as the standard,and the above two methods were used for diagnosis.Then,two authoritative chief physicians of imaging and two chief physicians of neurosurgery compared the results of the two diagnostic methods with the actual observation,and analyzed the anatomical relationship of neurovascular tissue.Results During the operation,there were 16 cases of type 1,9 cases of type 2 and 15 cases of type 3.MRI 3 D-TOF examination alone:11 cases of type 1,21 cases of type 2 and 8 cases of type 3;MRI 3 D-TOF combined with 3 D reconstruction showed 11 cases of type 1,21 cases of type 2 and 8 cases of type 3.There is a significant difference(P<0.05)between MRI 3 D-TOF examination and intraoperative observation,and the difference is statistically significant.However,there is no significant difference between MRI 3 D-TOF combined with 3 D reconstruction and intraoperative observation(P>0.05).Conclusion Combining 3 D reconstruction with 3 D-TOF sequence before microvascular decompression can comprehensively evaluate the lesion morphology and anatomical relationship of patients with trigeminal neuralgia,which has guiding significance for clinical diagnosis and treatment.3 D reconstruction is of great significance for the diagnosis of patients with primary trigeminal pain,and is worthy of further development and application.
