摘要
目的对比高龄老年人股骨颈骨折予以全髋与半髋关节置换术的疗效。方法方便选择该院2018年1月—2019年1月高龄老年人股骨颈骨折患者共350例,根据治疗方法分组,对照组的患者给予半髋关节置换术治疗,观察组给予全髋关节置换术治疗。比较两组术中总出血、操作总时间、住院总日数、治疗前后患者生存质量SF-36评分、髋关节功能评分、并发症发生率。结果观察组术中总出血(379.16±122.21)mL、操作总时间(91.21±6.01)min均高于对照组(236.56±51.45)mL、(64.78±4.44)min,差异有统计学意义(t=14.227、46.792,P<0.05);观察组生存质量SF-36评分(93.45±2.23)分、髋关节功能评分(96.65±2.93)分显著高于对照组(83.32±2.03)、(81.13±1.21)分,差异有统计学意义(t=44.438、64.766,P<0.05);观察组并发症发生率2.86%低于对照组9.14%,差异有统计学意义(χ^(2)=6.130,P<0.05)。结论高龄老年人股骨颈骨折予以全髋与半髋关节置换术,其中,半髋关节置换术的创伤比较轻,术中出血少,手术时间短,但是人工全髋关节置换术术后并发症更少,可更好改善患者的髋关节功能和生活质量,临床可根据患者的情况选择合适的手术方式。
Objective To compare the curative effect of total hip and hemi-hip arthroplasty for femoral neck fractures in the elderly.Methods From January 2018 to Janaury 2019,a total of 350 elderly patients with femoral neck fractures in the hospital were conveniently selected and grouped according to the treatment method.The patients in the control group were treated with hemi-hip replacement,and the observation group was treated with total hip replacement.The total bleeding,total operation time,total days of hospitalization,SF-36 score,hip function score,and complication rate of patients before and after treatment were compared between the two groups.Results The total intraoperative bleeding of the observation group was(379.16±122.21)mL and the total operation time(91.21±6.01)min were higher than those of the control group(236.56±51.45)mL,(64.78±4.44)min,the difference was statistically significant(t=14.227,46.792,P<0.05);the observation group survived quality SF-36 score(93.45±2.23)points and hip joint function score(96.65±2.93)points were significantly higher than those of the control group(83.32±2.03)points,(81.13±1.21)points,the difference was statistically significant(t=44.438,64.766,P<0.05);the observation group of the complication rate of 2.86%was lower than that of the control group 9.14%,the difference was statistically significant(χ^(2)=6.130,P<0.05).Conclusion The total hip and hemi-hip arthroplasty for elderly patients with femoral neck fractures,among them,hemi-hip arthroplasty has less trauma,less intraoperative bleeding and shorter operation time,but artificial total hip arthroplasty had fewer post-complications,which can better improve the patient's hip joint function and quality of life.Clinically,suitable surgical methods can be selected according to the patient's condition.
