摘要
为探讨超声内镜对结直肠黏膜高级别上皮内瘤变(HGIN)术前诊断的价值,选取2019年2月至2020年1月商丘市第四人民医院消化内科收治的结直肠黏膜活检病理结果为HGIN的97例患者,术前均行超声内镜检查,并依据检查结果行黏膜下剥离术或外科手术治疗。以术后病理学检查结果为准,判断超声内镜诊断的准确率。结果显示,1)超声内镜对病变浸润深度的正确诊断结果:腺瘤5例,黏膜内癌22例,黏膜下层癌7例,固有层癌14例,浆膜层癌39例。超声内镜对病变浸润深度的诊断准确率为89.69%(87/97)。2)术前超声内镜诊断单纯性HGIN 5例,早期结直肠癌23例,其中20例行内镜下黏膜剥离术,8例行外科手术治疗,25例超声内镜诊断与术后病理诊断符合;诊断进展期结直肠癌69例,均接受外科手术治疗,其中66例超声内镜诊断与术后病理诊断符合。超声内镜对97例结直肠HGIN患者的诊断准确性为93.81%(91/97)。结果表明,超声内镜检查用于活检病理结果为结直肠黏膜HGIN患者的术前诊断效果较好,对病变浸润深度的诊断准确性较高,有助于术者选择合适的手术方式,具有一定的指导意义。
This study was to investigate the value of endoscopic ultrasonography(EUS)in preoperatively diagnosing high-grade intraepithelial neoplasia(HGIN),enrolled 97 HGIN patients who had been confirmed HGIN by biopsy pathological exam.,and treated in author’s hospital(Dept.of digestive medicine)from 2019-02 to 2020-01:before surgery the patients all received EUS inspection,and according to the obtained results were performed submncous dieresis on them or treated surgically;then,taking postoperative pathologic exam.results as standard to judge accuracy rate of EUS diagnosis.As results,1)Here were right diagnosis on infiltration depth of lesion:adenoma of 5 cases,intramucous cancer of 22 cases,submucosal cancer of 39 cases,thus the accuracy rate was up to 86.69%(87/97);2)the diagnosis results before surgery:simple HGIN of 5 cases,early colorectal cancer of 23 cases,including performed dieresis on 20 cases,surgery on 8 cases,25 cases’ EUS diagnosis accorded with postoperative pathologic diagnosis;69 cases with advanced cancer all received surgical treatment in whom 66 cases’ EUS diagnosis accorded with postoprative pathologic diagnosis,thus the diagnostic accuracy rate of EUS on HGIN patients of 97 cases was up to93.81%(91/97).Results show that EUS has better preoperatively diagnosing efficacy,also,higher accuracy of diagnosing lesion’s infiltration depth,thus is helpful to direct operator choosing right procedure.
