摘要
本研究探讨磁共振弥散成像(DWI)与超声弹性成像(UE)在前列腺穿刺活检中的应用价值。112例疑似前列腺癌(PCa)患者中,经前列腺活检病理穿刺证实为阳性(PCa)59例、阴性(前列腺良性增生)53例。经研究发现,前列腺穿刺活检穿刺阳性患者超声弹性评分高于阴性患者,表观扩散系数(ADC)值低于阴性患者,二者均为前列腺穿刺活检阳性的影响因素,与前列腺穿刺活检阳性显著相关(P<0.05);ROC曲线显示,超声弹性评分、ADC值联合预测前列腺穿刺活检阳性的AUC面积为0.859,高于单一预测;前列腺穿刺活检阳性患者超声弹性评分、ADC值与Gleason评分相关(P<0.05)。DWI、UE对PCa均有较好的诊断能力,联合检查可提高PCa穿刺检出率,并能为肿瘤恶性程度及预后评估提供依据。
The purpose of this study is to explore the application value of diffusion weighted imaging(DWI)and ultrasonic elastography(UE)in prostate biopsy.Among 112 patients with suspected prostate cancer(PCa),59 were positive(PCa)confirmed by prostate biopsy and 53 were negative(benign prostatic hyperplasia).The study found that the ultrasound elasticity score of patients with positive prostate biopsy was higher than that of negative patients,and the apparent diffusion coefficient(ADC)value was lower than that of negative patients.Both were influencing factors for the positive prostate biopsy,and were significantly related to the positive prostate biopsy(P<0.05).The ROC curve showed that the combination of ultrasound elasticity score and ADC value predicted AUC to be 0.859,which was higher than the single prediction.In addition,the ultrasound elasticity score and ADC value of patients with positive prostate biopsy were correlated with Gleason score(P<0.05).It can be seen that both DWI and UE have good diagnostic capabilities for PCa,and combined examinations can improve the detection rate of PCa by puncture,and can provide a basis for the evaluation of malignant degree and prognosis of tumor.
