摘要
目的探讨临床药师参与的“糖尿病综合门诊”团队对门诊糖尿病患者血糖、血脂、血压管理达标率及用药依从性的影响。方法对2016-2019年在“糖尿病综合门诊”就诊且接受药物治疗的患者进行干预前后对照试验,由团队提供病情评估、诊断、治疗、用药教育、饮食及运动指导等一站式诊疗服务;比较患者初诊和随访1年后的治疗效果及用药依从性的改善情况。结果在“糖尿病综合门诊”就诊及随访1年后,患者的空腹血糖(FBG)、餐后血糖(PBG)、糖化血红蛋白(HbA1c)、血脂(TC/LDL-C/HDL-C)、血压、用药依从性均有明显改善(P<0.05),且血糖、血脂、血压同时达标人数增加(P<0.05)。结论临床药师参与的“糖尿病综合门诊”对糖尿病患者的综合治疗具有显著的改善效果,能提升患者综合达标率及用药依从性。糖尿病综合门诊在门诊诊疗中的开展及实践具有一定的实际意义。
Objective To explore the effect of the“diabetes comprehensive clinic”with clinical pharmacists participating on the management of blood glucose,blood lipid and blood pressure and medication compliance of outpatients with diabetes.Methods The before-and-after self-intervention control experiment was used.Patients who received drug treatment in the“comprehensive diabetes clinic”and were followed up for more than 1 year from 2016 to 2019 were selected as the study subjects.The“comprehensive diabetes clinic”team provides one-step services such as disease assessment,diagnosis,treatment,medication education,and diet and exercise guidance.The therapeutic effect and medication compliance were compared between the first visit to the diabetes clinic and the follow-up after 1 year.Results After 1 year of follow-up,the patient′s fasting blood glucose(FBG),postprandial blood glucose(PBG),glycated hemoglobin(HbA1c),blood lipids(TC/LDL-C/HDL-C),blood pressure and medication compliance were significantly improved(P<0.05),and the number of people whose blood glucose,blood lipids,and blood pressure all reached the standards simultaneously increased(P<0.05).Conclusion“Comprehensive diabetes clinic”with dinical pharmacists participating has a significant improvement effect in the comprehensive management of diabetic patients.It can increase the number of people reaching the standards and improve the medication compliance of patients.The development and practice of the“comprehensive diabetes clinic”in the outpatient clinic has certain practical significance.
