摘要
目的:观察川芎平喘汤加减对慢性阻塞性肺疾病(COPD)稳定期伴肺动脉高压(PAH)患者血清生长因子、外周血中性粒细胞/淋巴细胞比值的影响。方法:将2018年1月~2020年1月范围内收治的94例COPD伴PAH患者随机分为对照组(47例)和观察组(47例),对照组给予西医综合治疗,观察组在此基础上再予川芎平喘汤加减口服治疗,持续治疗2周。检测血流动力学[肺动脉收缩压(PASP)、平均肺动脉压(mPAP)]、肺功能[用力肺活量(FVC)、第1s用力呼气容积与用力肺活量比值(FEV1/FVC)、最大呼气峰流速(PEF)]、血清血管内皮生长因子(VEGF)、碱性成纤维细胞生长因子(bFGF)以及外周血中性粒细胞/淋巴细胞比值(NLR)变化。结果:与治疗前比较,两组PASP、mPAP降低(P<0.01),FVC、FEV1/FVC、PEF升高(P<0.01),血清VEGF、bFGF及外周血NLR降低(P<0.01);与对照组比较,观察组PASP、mPAP较低(P<0.01),FVC、FEV1/FVC、PEF较高(P<0.01),血清VEGF、bFGF及外周血NLR较低(P<0.05),临床疗效较佳(P<0.05)。结论:川芎平喘汤加减治疗COPD伴PAH疗效确切,与调节血清VEGF、bFGF及外周血NLR有关。
Objective: To investigate the effect of Modified Chuanxiong Pingchuan Decoction on serum growth factor and peripheral blood neutrophil/lymphocyte ratio for patients with chronic obstructive pulmonary disease(COPD) in stable phase and pulmonary hypertension(PAH).Methods: 94 patients with COPD and PAH from January 2018 to January 2020 were randomly divided into controlled group(47 cases) and observation group(47 cases).The controlled group received comprehensive western medicine treatment, the observation group received more with Modified Chuanxiong Pingchuan Decoction, and 2 groups were treated for 2 weeks.Hemodynamics(PASP,mPAP),pulmonary function(FVC,FEV1/FVC,PEF),VEGF,bFGF and NLR) were measured.Results: Compared with the controlled group, PASP and mPAP decreased(P<0.01),FVC,FEV1/FVC and PEF increased(P<0.01),serum VEGF,bFGF and peripheral blood NLR decreased(P<0.01),compared with the controlled group, PASP and mPAP in the observation group were lower(P<0.01),FVC,FEV1/FVC and PEF were higher(P<0.01),serum VEGF,bFGF and peripheral blood NLR were lower(P<0.05),and the clinical effect was better(P<0.05).Conclusion: Modified Chuanxiong Pingchuan Decoction is effective in the treatment of COPD with PAH,which is related to the regulation of VEGF,bFGF and NLR.
