Periostin在老年股骨转子间骨折治疗中的影响和意义

Effect and Significance of Periostin in the Treatment of Femoral Intertrochanteric Fracture in the Elderly

摘要 目的:通过研究血清骨膜蛋白(periostin)在老年女性股骨转子间骨折治疗过程中的变化,分析骨膜蛋白在骨折及骨质疏松患者治疗过程中的影响和意义。方法:纳入笔者所在医院2018年1月-2020年3月收治的36例老年女性股骨转子间骨折患者作为骨折组,36例老年女性骨质疏松患者作为骨质疏松组,分别检测两组患者治疗前、治疗第1、2、3、6个月血清骨膜蛋白水平、血清骨硬化蛋白水平及腰椎骨密度水平。结果:两组治疗前血清骨膜蛋白水平、骨硬化蛋白水平及腰椎骨密度水平差异均无统计学意义(P>0.05);骨折组治疗第1、2、3、6个月,血清骨膜蛋白水平呈总体上升趋势,均显著高于治疗前及骨质疏松组治疗后水平(P<0.05);骨折组血清骨硬化蛋白水平呈下降趋势,均较治疗前显著降低,且明显低于骨质疏松组水平(P<0.05);骨折组治疗后腰椎骨密度水平呈现上升趋势,其中骨折组第3、6个月腰椎骨密度水平较治疗前和同期骨质疏松组均偏高(P<0.05);骨折组治疗第6个月,骨膜蛋白与骨密度呈正相关(r=0.568,P<0.01),与骨硬化蛋白呈负相关(r=-0.676,P<0.01);骨硬化蛋白和骨密度之间呈显著负相关(r=0.395,P<0.05)。结论:骨膜蛋白在骨折患者血清中的表达水平较高,随着患者病情的康复,其表达水平明显升高,且与骨硬化蛋白水平呈负相关,有望成为股骨转子间骨折患者病情监测的参考指标,如能协同骨硬化蛋白则意义更大,值得推广。 Objective:By studying the changes of serum periosteal protein in the treatment process of intertrochanteric fracture of femur in elderly women,the influence and significance of periosteal protein in the treatment process of fracture and osteoporosis patients were analyzed.Method:Thirty-six elderly female patients with intertrochanteric fracture as the fracture group and 36 elderly female patients with osteoporosis treated in our hospital from January 2018 to March 2020 were enrolled as osteoporosis group.The levels of serum periosteal protein,serum osteoprotegerin and lumbar bone mineral density were measured before treatment and at the first,third and sixth months after treatment.Result:There was no significant difference in serum periosteal protein level,osteoprotegerin level and lumbar bone mineral density between the two groups before treatment (P>0.05).After treatment for 1,2,3 and 6 months,the level of serum periosteal protein in fracture group showed an upward trend,which was significantly higher than that before treatment and after treatment in osteoporosis group (P<0.05).The level of serum osteoprotegerin in fracture group showed a downward trend,which was significantly lower than that before treatment and significantly lower than that in osteoporosis group (P<0.05).After treatment,the bone mineral density level of lumbar vertebrae in fracture group showed an upward trend,and the bone mineral density level of lumbar vertebrae in fracture group at the third and sixth months was higher than that before treatment and osteoporosis at the same period (P<0.05).After 6 months of treatment,periosteal protein was positively correlated with bone mineral density (r=0.568,P<0.01) and negatively correlated with osteoprotegerin (r=-0.676,P<0.01).There was a significant negative correlation between osteoprotegerin and bone mineral density (r=0.395,P<0.05).Conclusion:The expression level of periosteal protein in the serum of patients with femoral intertrochanteric fracture is higher.With the recovery of patients’ cond
