摘要 目的通过使用Revolution CT对肺结节患者进行GSI平扫及动、静脉期双期增强扫描,并利用后处理软件进行能谱的多参数定量分析,判断Revolution CT能谱成像技术对肺部炎性与恶性结节鉴别诊断及肺癌病理分型的价值。方法对2018年3月至2018年9月于蚌埠医学院第一附属医院行Revolution CT肺部GSI平扫及动、静脉期双期增强扫描并符合纳入条件的86例肺部结节患者进行能谱多参数定量分析,并对炎性结节与恶性结节及腺癌与鳞癌的多种能谱参数进行统计学分析,使用的统计学方法为两个独立样本t检验,P<0.05为差异有统计学意义。结果炎性结节的平扫Eff-Z、平扫与动脉期及静脉期的K、动脉期及静脉期的CT_(40ke V)、动脉期及静脉期的ΔCT_(40ke V)、动脉期及静脉期的NIC均高于恶性结节,且差异有统计学意义(P<0.05);腺癌的Eff-Z、平扫与动脉期及静脉期的K、动脉期及静脉期的CT_(40ke V)、动脉期及静脉期的ΔC_(T40ke V)、动脉期及静脉期的NIC均高于鳞癌,且差异有统计学意义(P<0.05)。结论 Revolution CT能谱成像技术结合其多参数定量分析对于肺部炎性结节与恶性结节的鉴别诊断以及肺癌的病理分型具有较高的诊断价值。 Objective The value of Revolution CT in the differential diagnosis of inflammatory and malignant pulmona ry nodules and the pathological classification of lung cancer was determined by GSI plain and dual-phase(arterial and venous phase)enhanced scanning of pulmona ry nodules and multi-parameter quantitative analysis of energy spectrum using data post-processing software.Methods From March2018 to September 2018,a total of 86 cases of pulmonary nodules eligible for inclusion in the first affiliated hospital of Bengbu medical college were subjected to the quantitative analysis of energy spectrum in terms of lung GSI plain and dual-phase(arterial phase and venous phase)enhanced scanning by Revolution CT.Moreover,statistical analysis was conducted on various energy spectrum parameters of inflammatory nodules,malignant nodules,adenocarcinoma,and squamous cell ca rcinoma.The statistical method was the t-test of two independent sa mples.P<0.05 was considered statistically significant.Results For the inflammatory nodules,the plain scan Eff-Z,the K values of plain scan and dual-phase(arterial phase and venous phase),CT40 keV of arterial phase and venous phase,ΔCT40 keV of arterial phase and venous phase,NIC of a rterial phase and venous phase,AIC of a rterial phase and venous phase were higher than malignant nodules,and the differences were statistically significant(P<0.05).2.For the adenocarcinoma cases,the plain scan Eff-Z,the K values of plain scan and dual-phase(a rterial phase and venous phase),CT40 keV of arterial phase and venous phase,ΔCT40 keVof arterial phase and venous phase,NIC of a rterial phase and venous phase,ΔIC of a rterial phase and venous phase were higher than that of squamous cell ca rcinoma,and the diffe rences we re statistically significant(P<0.05).Conclusion Revolution CT energy spectrum imaging combined with its multiparameter quantitative analysis has a high diagnostic value for the differential diagnosis of pulmona ry inflammatory nodules and malignant nodules as well as the pathological cla
