薄层CT引导下碘125粒子联合化疗治疗中晚期非小细胞肺癌临床效果观察 被引量:6

摘要 目的探讨薄层CT引导下碘125(^(125)I)粒子联合化疗治疗中晚期非小细胞肺癌(NSCLC)的临床效果。方法选取北部战区总医院自2016年1月至2017年10月收治的85例中晚期NSCLC患者为研究对象。根据不同的治疗方法将其分为A组(n=40)与B组(n=45)。A组患者单纯给予化疗治疗;B组患者在薄层CT引导下,经皮穿刺植入^(125)I粒子,并于2周后给予化疗治疗。比较两组患者治疗的有效率。记录并比较两组患者1、2年的生存率,治疗前后血清细胞角蛋白19片段(CYFRA21-1)、癌胚抗原(CEA)、鳞状细胞癌相关抗原(SCC-Ag)水平,卡氏评分(KPS),以及不良反应发生情况。结果B组患者治疗的有效率为73.33%(33/45),高于A组的45.00%(18/40),差异有统计学意义(P<0.05)。B组患者1、2年的生存率均高于A组,差异均有统计学意义(P<0.05)。治疗后,B组患者的KPS为(78.36±4.13)分,高于A组的(74.28±2.41)分,差异有统计学意义(P<0.05)。结论薄层CT引导下,^(125)I粒子联合化疗治疗中晚期NSCLC临床疗效确切,能明显降低血清肿瘤标志物水平,并提高患者的生存质量。 Objective To investigate the clinical efficacy of Iodine-125(^(125)I)seeds implantation combined with chemotherapy guided by thin-section CT in the treatment of advanced non-small cell lung cancer(NSCLC).Methods A retrospective study was performed on 85 cases of patients with advanced NSCLC who were admitted from January 2016 to October 2017.According to different treatment methods,patients were divided into the Group A(n=40)and Group B(n=45).Group A of patients were performed single chemotherapy;Group B of patients were implanted with I125 seeds through percutaneous puncture under the guidance of thin-section CT,and chemotherapy was given 2 weeks later.The effective rate of treatment was compared between the two groups.The 1 and 2 years survival rate,serum levels of tumor marker cytokeratin 19 fragment(CYFRA21-1),carcinoembryonic antigen(CEA),squamous cell carcinoma associated antigen(SCC-Ag),Karnofsky scores(KPS)and the incidence of ADR were recorded and compared between two groups before and after treatment.Results The effective rate of Group B was 73.33%(33/45),which was higher than that of Group A(45.00%,18/40),and the difference was statistically significant(P<0.05).The 1 and 2-year survival rates in Group B were all higher than those in Group A(P<0.05).After treatment,the KPS of Group B was(78.36±4.13)points,which was higher than that of Group A(74.28±2.41)points,P<0.05.Conclusion I^(125) seeds implantation combined with chemotherapy has definite clinical efficacy in the treatment of advanced NSCLC,which can significantly reduce the level of serum tumor markers and improve the quality of life of patients.
