阿尔茨海默病血代谢指标及生物标志物的相关性研究 被引量:1

Study on the correlation between blood metabolic indexes and biomarkers in Alzheimer’s disease

摘要 目的探讨阿尔茨海默病(Alzheimer’s disease,AD)患者血代谢指标变化及与疾病发生发展的关系,寻找其与脑脊液生物标记物的相关性。方法纳入2018年9月-2020年7月诊断为AD的病例66例,以及同时期年龄性别无差异的正常对照36例,检测所有受试者血清代谢指标水平;测定并比较患者脑脊液tau蛋白、淀粉样蛋白(Amyloid,Aβ)水平,与代谢指标进行相关性分析。结果AD组的同型半胱氨酸(Homocysteine,HCY)、高密度脂蛋白胆固醇(high density lipoprotein cholesterol-C,HDL-C)、叶酸和维生素B12(Vitamin B12,VitB12)与对照组有明显差异(P<0.05);多因素回归分析校正后显示HDL-C、VitB12与AD的发生明显相关;HCY、HDL-C、叶酸和VitB12水平与认知评分的Pearson相关性分析有明显意义(P<0.05);AD组患者脑脊液tau蛋白水平显著高于对照组,Aβ42浓度明显低于对照组;代谢差异指标与其进行Pearson相关性分析中显示HDL-C与tau蛋白水平呈正相关,VitB12水平与Aβ42之间存在正相关(P<0.05)。结论本研究表明,AD患者认知损害程度与HCY及HDL-C水平呈正相关,叶酸和VitB12水平与AD认知障碍严重程度呈负相关,或为AD的保护性因素。 Objective To explore the relationship between the blood metabolic indexes and the occurrence and development of Alzheimer’s disease(AD)and to find the correlation with cerebrospinal fluid biomarkers.Methods A total of 66 patients diagnosed with AD from September 2018 to July 2020 and 36 normal controls with no difference in age and gender were enrolled in this study.The levels of serum metabolic indexes were detected in all subjects.The levels of tau protein and amyloid protein(Aβ)in cerebrospinal fluid were measured and compared.and the correlation between tau protein and metabolic indexes was analyzed.Results The levels of homocysteine(Hcy),high density lipoprotein cholesterol(HDL-C),folic acid and Vitamin B12(VitB12)in AD group were significantly different from those in control group(P<0.05);after adjustment of multivariate regression analysis.HDL-C and VitB12 were significantly correlated with the occurrence of AD;the levels of HCY,HDL-C.folic acid and VitB12 were significantly correlated with cognitive score in Pearson correlation analysis(P<0.05).The level of tau protein in cerebrospinal fluid was significantly higher than that in the control group.and the concentration of Aβ42 was significantly lower than that in the control group.Pearson correlation analysis showed that HDL-C was positively correlated with tau protein level.and VitB12 was positively correlated with Aβ42(P<0.05).Conclusion This study shows that the degree of cognitive impairment in AD patients is positively correlated with HCY and HDL-C levels.and the levels of folic acid and VitB12 are negatively correlated with the severity of cognitive impairment in AD patients.Which may be protective factors of AD.
