摘要
目的探讨早期适时肠内营养支持在重症急性胰腺炎(SAP)患者中的应用效果。方法收集2018年2月至2019年8月在本院接受诊治的200例SAP患者作为研究对象,根据患者肠内营养支持启动时机不同将其分为早期组(110例,早期适时肠内营养支持)、延迟组(90例,延迟肠内营养支持)。比较两组患者的免疫功能指标、炎症反应指标、营养状况指标及并发症发生情况。结果干预后,早期组的IgA、IgG、IgM水平及CD3^(+)、CD4^(+)、CD3^(+)/CD4^(+)均显著高于延迟组(P<0.05)。干预后,早期组的IL-6、TNF-α、CRP、AMS水平低于干预前及延迟组,Alb、PA水平高于干预前及延迟组(P<0.05)。早期组肠麻痹、菌血症、腹腔感染、肺功能不全、多器官功能障碍综合征发生率均低于延迟组(P<0.05)。结论早期适时肠内营养支持用于SAP患者中,可增强患者的免疫功能,减轻炎症反应,改善机体营养状态,且并发症较少。
Objective To evaluate the application effect of early timely enteral nutrition support in patients with severe acute pancreatitis.Methods A total of 200 SAP patients treated in our hospital from February 2018 to August 2019 were selected as the research objects.The patients were divided into early group(110 cases,early timely enteral nutrition support)and delayed group(90 cases,delayed enteral nutrition support)according to different timing of enteral nutrition support initiation.The immune function indexes,inflammatory reaction indexes,nutritional status indexes and complications of the two groups were compared.Results After intervention,the IgA,IgG,IgM levels and CD3^(+),CD4^(+),CD3^(+)/CD4^(+)in the early group were significantly higher than those in the delayed group(P<0.05).After intervention,the levels of IL-6,TNF-α,CRP and AMS in the early group were lower than those before intervention and the delayed group,while the levels of ALB and PA were higher than those before intervention and the delayed group(P<0.05).The incidence of intestinal paralysis,bacteremia,abdominal infection,pulmonary insufficiency and multiple organ dysfunction syndrome in the early group were lower than those in the delayed group(P<0.05).Conclusion Early timely enteral nutrition support can enhance the immune function of patients with SAP,reduce the inflammatory reaction,improve the nutritional status of the body,and has fewer complications.
