摘要
目的先天性甲状腺功能减退作为一种先天性疾病,严重影响患儿智力发育和生长发育,及早发现和及时治疗可以有效降低患儿智力发育及生长发育障碍。本研究探究早期康复疗法对先天性甲状腺功能减退儿童生长发育的影响,为临床治疗提供参考。方法选择2016-01-01-2018-01-01于南阳市第一人民医院出生并接受治疗的96例先天性甲状腺功能减退患儿为研究对象,根据性别、胎龄和出生方式匹配的原则选择观察组与对照组。观察组51例进行早期康复疗法,对照组45例进行常规干预疗法。比较两组患者干预前后身高、体质量和平均头围等体格发育数据;比较两组患者干预前后促甲状腺激素(thyroid stimulating hormone,TSH)、游离三碘甲状腺原氨酸(free triiodothyronine 3,FT3)和血清游离甲状腺素(serum free thyroxine,FT4)水平;比较两组患者治疗前后认知功能、语言功能、适应行为、大动作能力和精细动作等发育商评分情况。结果治疗后两组患儿身高、体质量和头围均有所增加,但组间差异无统计学意义,均P>0.05。治疗后观察组TSH水平为(3.12±0.42)μIU/mL,对照组为(3.21±0.45)μIU/mL,差异无统计学意义,t=1.143,P=0.276;FT3水平为(8.39±2.55)pmol/L,对照组为(7.68±2.19)pmol/L,差异无统计学意义,t=1.478,P=0.212;FT4水平为(24.19±7.29)pmol/L,对照组为(23.33±7.48)pmol/L,差异无统计学意义,t=1.539,P=0.179。治疗后观察组认知功能评分为(98.22±3.67)分,高于对照组的(83.10±2.58)分,t=2.398,P=0.026;语言功能评分为(97.19±4.11)分,高于对照组的(76.83±3.70)分,t=2.360,P=0.031;适应行为评分为(93.47±2.17)分,高于对照组的(81.22±1.98)分,t=2.649,P=0.012;大动作能力评分为(95.44±3.67)分,高于对照组的(74.32±3.31)分,t=2.448,P=0.021;精细动作能力评分为(91.21±4.57)分,高于对照组的(77.32±3.91)分,t=2.761,P=0.007。结论早期康复疗法可以促进先天性甲状腺功能减退患儿的智�
OBJECTIVE Congenital hypothyroidism,as a congenital disease,seriously affectschildren’s intellectual development and growth.Early detection and timely treatment can effectively reduce children’s intellectual development and growth disorders.This study explored the effect of early rehabilitation therapy on growth and development of children with congenital hypothyroidism,and provided references for clinical treatment.METHODS A total of 96 children with congenital hypothyroidism who were born and treated by January 1,2016 to January 1,2018 were selected as the subjects.The observation group and the control group were selected according to the principle of sex,gestational age and birth pattern matching.51 cases in the observation group received early rehabilitation therapy,while 45 cases in the control group received routine intervention therapy.The height,body mass and average head circumference of the two groups before and after intervention were compared.The levels of thyroid stimulating hormone(TSH),free triiodothyronine 3(FT3)and serum free thyroxine(FT4)before and after intervention were compared;the cognitive and linguistic functions of the two groups before and after treatment were compared;the developmental quotient scores of adaptive behavior,large motor ability and fine motor ability were evaluated.RESULTS After treatment,the height,weight and head circumference of the children increased,but there was no significant difference between the two groups(P>0.05).After treatment,the TSH level in the observation group was(3.12±0.42)IU/ml,and that in the control group was(3.21±0.45)IU/ml,t=1.143,P=0.276,the difference was not statistically significant;the FT3 level was(8.39±2.55)pmol/L,and the control group was(7.68±2.19)pmol/L,t=1.478,P=0.212,the difference was not statistically significant;the FT4 level was(24.19±7.29)pmol/L,and the control group was(23.33±7.48)pmol/L,t=1.539,P=0.179,the difference was not statistically significant.After treatment,the cognitive function score of the observation group
