摘要
[目的]探讨活络效灵丹在下肢静脉曲张腔内激光治疗(EVLT)术后隐神经损伤中的应用价值。[方法]选择2018年1月—2019年10月在上海中医药大学附属普陀医院经下肢静脉造影确诊的60例下肢大隐静脉曲张患者为研究对象,所有患者均有疼痛、麻木等隐神经损伤症状,运用随机数字表法将其分成两组,每组30例。其中观察组采取马栗种子提取物片联合活络效灵丹治疗,对照组则予以马栗种子提取物片联合甲钴胺片治疗,所有对象均连续治疗1个月后停用马栗种子提取物片,其余药物再连用2个月,疗程共3个月。比较两组临床疗效及安全性。治疗前后,采用“主诉疼痛分级法”对受试者的主观疼痛程度进行量化评估,测定患肢隐神经传导速度,评价生活质量[36项健康调查简表(SF-36)]。[结果]观察组总有效率[93.33%(28/30)]较对照组[60.00%(18/30)]差异有统计学意义(P<0.05)。治疗后,两组主观疼痛发生率比较,差异无统计学意义(P>0.05)。随治疗时间的延长,两组患肢隐神经传导速度均逐渐提高(P<0.05),且观察组治疗1、3个月时的患肢隐神经传导速度均显著高于对照组同期(P<0.05)。两组SF-36评分均随治疗时间的延长而逐渐增高(P<0.05),但观察组治疗1、3个月时的SF-36评分较对照组同期均显著更高(P<0.05)。两组均无明显不良事件发生。[结论]活络效灵丹能有效促进下肢静脉曲张患者EVLT术后受损隐神经的恢复,改善生命质量,且安全性较高。
[Objective]To explore the application value of Huoluo Xiaoling Dan on saphenous nerve injury after endovenous laser treatment(EVLT).[Methods]From January 2018 to October 2019,60 patients with varicose veins of the great saphenous vein of lower limbs were selected as the research objects.All patients had pain,numbness and other symptoms of saphenous nerve injury,and were divided into two groups with 30 cases in each group by random number table method.The observation group was treated with horse chestnut seed extract tablets combined with Huoluo Xiaoling Dan,while the control group was treated with horse chestnut seed extract tablets combined with mecobalamine tablets.All subjects stopped using horse chestnut seed extract tablets after continuous treatment for one month,and the other drugs were used for another two months,with a course of treatment of three months.The clinical efficacy and safety of two groups were compared.Before and after treatment,the subjective pain degree of the subjects was quantitatively evaluated by the“subjective pain rating method”,the saphenous nerve conduction velocity of the affected limb was measured,and the quality of life was evaluated[36-item health survey short-form(SF-36)].[Results]The total effective rate of the observation group[93.33%(28/30)]was significantly higher than that of the control group[60.00%(18/30)](P<0.05).After treatment,there was no significant difference in the incidence of subjective pain between two groups(P>0.05).With the prolongation of treatment time,the conduction velocity of saphenous nerve increased gradually in both groups(P<0.05).Moreover,the saphenous nerve conduction velocity of the affected limb in the observation group was significantly higher than that in the control group at the same time(P<0.05).The scores of SF-36 in two groups increased gradually with the prolongation of treatment time(P<0.05),but the scores of SF-36 in the observation group were significantly higher than those in the control group during the same period(P<0.05).No obvio
