摘要
一例胸痛患者突发意识丧失,心电活动呈顽固性室颤 (VF),结合心电图动态改变,诊断为急性前壁ST段抬高型心肌梗死。经反复电除颤35次,联合可达龙、10%氯化钾、25%硫酸镁、艾司洛尔等药物静脉注射,并经溶栓、经皮冠状动脉介入治疗 (PCI)后终止VF,病情恢复良好。随访至今,无不良心血管事件发生。一例诊断为急性前壁ST段抬高型心肌梗死的患者,成功行PCI治疗。术后第4天20:34、23:24,第5天08:06三次突发意识丧失,全身抽搐,心电监护示:室颤。每次给予360J心脏电除颤后终止VF,经25%硫酸镁静脉注射治疗后再未复发,随访至今,无不良心血管事件发生。
1 patient with chest pain had a sudden loss of consciousness,whose cardiac electrical activity had refractory ventricular fibrillation (VF).Acute anterior ST segment elevation myocardial infarction was diagnosed by ECG dynamic changes.Received repeated electrical defibrillation 35 times,combined with Intravenous injection of cordarone,10% potassium chloride,25% magnesium sulfate,esmolol and other drugs,as well as thrombolytic and percutaneous coronary intervention (PCI),VF was terminated and the patient recovered well.The patient was followed up,who had no adverse cardiovascular event but improved cardiac function.1 patient was diagnosed with acute anterior ST segment elevation myocardial infarction and received successful PCI treatment.The 4 day 20:34,23:24,the 5 day 08:26 after surgery,the patient had 3 times of sudden loss of consciousness with generalized spasm.ECG monitoring showed ventricular fibrillation.VF was terminated after 360J Cardiac Defibrillation every times.VF was not relapse after intravenous treatment with 25% magnesium sulfate.The patient was followed up,who had no adverse cardiovascular event but improved cardiac function.
