动态血糖监测在2型糖尿病血糖监控效果分析 被引量:4

摘要 目的应用动态血糖监测系统(continuous glucose monitoring system,CGMS)与以往指尖毛细血管末梢血糖监测对2型糖尿病精确性的差异,探讨CGMS在发现低血糖中的临床价值。方法选取2019年10月至2020年2月收治入院的2型糖尿病患者70例,其中男36例,年龄范围为24~75岁,年龄(46.8±8.3)岁;女34例,年龄范围为29~71岁,年龄(40.2±9.2)岁。入院后72 h同时用血糖仪进行毛细血管血糖及CGMS监测,5次/d(3餐前、夜间10:00及凌晨),同时收集疾病相关资料,分析糖尿病患者低血糖的发生时段特点及2种测量方式的差异性。结果三餐前及凌晨CGMS血糖值分别为(7.50±1.74)mmol/L、(9.74±2.85)mmol/L、(9.45±2.53)mmol/L、(7.83±2.11)mmol/L,均低于指尖毛细血管末梢血糖(7.80±1.78)mmol/L、(11.41±3.13)mmol/L、(10.06±2.94)mmol/L、(8.24±2.13)mmol/L,差异均有统计学意义(均P<0.05);夜间10∶00所测得CGMS血糖值略高于指尖毛细血管末梢血糖值,但差异无统计学意义(P>0.05);各时段均有低血糖现象的发生,以凌晨时段最多,总体观察CGMS监测出的例数较常规指尖监测的多,其中以凌晨时段发生低血糖的病例最多,CGMS凌晨测量出的低血糖检出率为18.57%(13/70),高于指尖毛细血管末梢血糖7.14%(5/70),差异有统计学意义(P<0.05);三餐前及夜间10:00的CGMS较指尖毛细血管末梢血糖测量出的低血糖数量上多,但差异均无统计学意义(均P>0.05)。结论CGMS可实时监控血糖情况,对低血糖的监测较传统指尖毛细血管末梢血糖检出效果更优越,可减少后期低血糖的发生。 Objective To explore the clinical value of continuous glucose monitoring system in detecting hypoglycemia in patients with type 2 diabetes mellitus by comparing the differences from fingertip capillary blood glucose meter.Methods Seventy patients with type 2 diabetes treated at our hospital from October,2019 to February,2020 were selected,including 36 males who were 24-75(46.8±8.3)years old and 34 females who were 29~71(40.2±9.2)years old.Capillary blood glucose was detected by glucose meter and the glucose level was monitored by the continuous glucose monitoring system continuously for 72 hours after admission,5 times a day(before meals,22∶00,and morning);meanwhile,their disease-related data were collected.The characteristics of the period of hypoglycemia in the patients and the differences between the two measurement methods were analyzed.Results The blood glucose levels before meals and in the early morning by the system were lower than those by the meter[(7.50±1.74)mmol/L vs.(7.80±1.78)mmol/L,(9.74±2.85)mmol/L vs.(11.41±3.13)mmol/L,(9.45±2.53)mmol/L vs.(10.06±2.94)mmol/L,and(7.83±2.11)mmol/L vs.(8.24±2.13)mmol/L;all P<0.05].The blood glucose level at 22:00 by the system was higher that by the meter,but with no statistical difference(P>0.05).Hypoglycemia occurred in all periods,mostly in early morning;and more cases were detected by the system than by the meter.The detection rate of hypoglycemia in the morning by the system was higher than that by the meter[18.57%(13/70)vs.7.14%(5/70),P<0.05].The detection rates of hypoglycemia before meals and at 22:00 were higher by the system than by the meter,but with no statistical differences(all P>0.05).Conclusion The continuous glucose monitoring system can monitor blood glucose in real time and reduce the occurrence of hypoglycemia in later period,and is more effective than the traditional fingertip capillary blood glucose detection.
