摘要
目的:对加味苓桂术甘汤治疗慢性心力衰竭的疗效及炎症因子水平的影响进行观察分析。方法:选取2018年12月—2020年1月重庆市垫江县中医院心血管内科住院部的慢性心力衰竭(阳虚水停证)患者60例,治疗组和对照组各30例。对照组予规范心力衰竭药物治疗,治疗组在西药治疗上加用加味苓桂术甘汤治疗。共治疗4周,对患者临床疗效、中医证候积分、左室射血分数(LVEF)值、氨基末端脑钠素原(NT-proBNP)水平、肿瘤坏死因子-α(TNF-α)水平和白细胞介素-6(IL-6)水平进行比较。结果:治疗后2组临床疗效均好转,治疗组有效率优于对照组;2组中医证候积分治疗后均明显下降,治疗组治疗后中医证候积分下降程度更大;2组治疗后LVEF均明显升高,治疗组治疗后LVEF升高程度更大;2组治疗后NT-proBNP、TNF-α、IL-6均明显降低,治疗组治疗后NT-proBNP、TNF-α、IL-6均下降程度更大,差异均有统计学意义。结论:加味苓桂术甘汤加西药治疗慢性心力衰竭,能有效改善临床疗效,提高LVEF,降低患者NT-proBNP、TNF-α、IL-6水平,临床效果明显优于单独使用常规西药治疗。
Objective:To observe and analyze the curative effect of Jiawei Linggui Zhugan decoction in the treatment of chronic heart failure and the influence of inflammatory factor levels.Methods:From December 2018 to January 2020,60 patients with chronic heart failure(Yang deficiency and water stop syndrome)in the inpatient department of the Department of Cardiology,Dianjiang County Hospital of Traditional Chinese Medicine,Chongqing City were selected.There were 30 cases in the treatment group and 30 cases in the control group.The control group was treated with standardized heart failure drugs,and the treatment group was treated with Western medicine plus Jiawei Lingguizhugan decoction.A total of 4 weeks of treatment,the patient’s clinical efficacy,TCM syndrome score,left ventricular ejection fraction(LVEF)value,N-terminal pro-brain natriuretic factor(NT-proBNP)level,tumor necrosis factor-α(TNF-α)level and Interleukin-6(IL-6)levels were compared.Results:After treatment,the clinical efficacy of the two groups improved,and the effective rate of the treatment group was better than that of the control group;the TCM syndrome scores of the two groups were significantly reduced after treatment,and the TCM syndrome scores of the treatment group decreased more after treatment;LVEF after treatment in the two groups LVEF increased significantly after treatment in the treatment group;NT-proBNP,TNF-α,IL-6 were significantly reduced after treatment in the 2 groups,and NT-proBNP,TNF-α,IL-6 in the treatment group after treatment The degree of decline was even greater,and the differences were statistically significant.Conclusion:Modified Linggui Zhugan decoction plus western medicine in the treatment of chronic heart failure can effectively improve the clinical efficacy,increase LVEF,and reduce the levels of NT-proBNP,TNF-α,and IL-6 in patients.The clinical effect is significantly better than that of conventional western medicine alone.
