负荷剂量替考拉宁治疗革兰阳性菌重症肺炎的临床观察

Effects of loading-dose teicoplanin on patients with severe Gram-positive pneumonia

摘要 目的探究负荷剂量替考拉宁治疗革兰阳性菌重症肺炎的临床效果。方法抽取2019年3月至2019年9月郑州大学附属郑州中心医院收治的革兰阳性菌重症肺炎患者258例,根据随机数字表法分为观察组(130例)与对照组(128例)。两组均给予替考拉宁进行治疗,观察组初始负荷剂量为400 mg/次,2次/d,持续3 d,随后给予维持剂量200 mg/次,1次/d;对照组给予常规剂量200 mg/次,1次/d。比较两组患者治疗3周后的微生物清除率和临床疗效,比较两组患者治疗前与治疗3周后血清肿瘤坏死因子-α(TNF-α)、C-反应蛋白(CRP)、降钙素原(PCT)水平。结果治疗3周后,观察组微生物清除率(82.50%,107/130)高于对照组(62.50%,80/128),P<0.05;观察组临床总有效率(98.46%,128/130)高于对照组(92.97%,119/128),P<0.05;观察组TNF-α、CRP、PCT水平低于对照组,差异有统计学意义(P<0.05)。结论负荷剂量替考拉宁可以提高革兰阳性菌重症肺炎患者的微生物清除率,有效降低患者血清TNF-α、CRP、PCT水平。 Objective To investigate the effects of loading-dose teicoplanin on patients with severe Gram-positive bacterial pneumonia.Methods A total of 258 patients with severe Gram-positive bacteria pneumonia admitted to Zhengzhou Central Hospital Affiliated to Zhengzhou University from March 2019 to September 2019 were enrolled,and they were divided into observation group(n=130)and control group(n=128)by random number table method.Patients of both groups were treated with teicoplanin.The observation group was given the initial loading dose of 400 mg/time,twice a day,lasting for 3 days,followed by the maintenance dose of 200 mg/time,once a day.The control group was given a conventional dose of 200 mg/time,once a day.The microbial clearance rate and clinical efficacy of the two groups were compared after 3 weeks of treatment.The levels of tumor necrosis factor-α(TNF-α),C-reactive protein(CRP),and procalcitonin(PCT)in serum in the two groups were compared before treatment and after 3 weeks of treatment.Results After 3 weeks of treatment,the microbial clearance rate of the observation group(82.50%,107/130)was significantly higher than that of the control group(62.50%,80/128),P<0.05.The total clinical efficacy rate of the observation group(98.46%,128/130)was significantly higher than that of the control group(92.97%,119/128)after 3 weeks of treatment,P<0.05.The levels of TNF-α,CRP,and PCT in the observation group were significantly lower than those in the control group after 3 weeks of treatment(P<0.05).Conclusions Using loading-dose teicoplanin can improve the microbial clearance rate and effectively reduce the levels of TNF-α,CRP and PCT in patients with severe Gram-positive bacterial pneumonia.
