摘要
目的探讨维持性血液透析(MHD)的慢性肾衰竭患者的心功能及相关指标。方法便利选取2016年1月—2019年12月于该院进行维持性血液透析的75例患者为研究对象,另选健康志愿者50名为正常组,检测两组的左室大小、左室收缩功能及血清中N-末端脑钠肽前体(NT-proBNP)、肌酐(Scr)和尿素氮(BUN)的水平,并进行相关性分析。结果观察组患者的NT-proBNP、Scr、BUN水平和左心室舒张末期内径(LVEDD)均高于对照组,差异有统计学意义(P<0.05);观察组患者左心室射血分数(LVEF)(53.07±14.33)%低于正常组(68.61±9.28)%,差异有统计学意义(t=8.518,P<0.05);维持性血液透析患者的LVEDD与NT-proBNP、Scr、BUN水平呈正相关(P<0.05),LVEF与NT-proBNP、BUN水平呈负相关(P<0.01)。结论MHD患者的心脏收缩和舒张功能降低,血清中NT-proBNP、Scr和BUN水平升高,其心脏结构和功能的改变与NT-proBNP、Scr和BUN的水平相关,MHD能够改善慢性肾衰竭患者的预后,为减少慢性肾衰竭患者的心脏结构和功能障碍提供思路。
Objective To investigate the cardiac function and related indexes in patients with chronic renal failure on maintenance hemodialysis(MHD).Methods 75 patients who underwent maintenance hemodialysis in the hospital from January 2016 to December 2019 were conveniently selected as the research objects,and 50 healthy volunteers were selected as the normal group,and the left ventricular size and left ventricular contraction of each group were measured.Function and serum N-terminal pro-brain natriuretic peptide(NT-proBNP),creatinine(Scr)and urea nitrogen(BUN)levels,and carry out correlation analysis.Results The levels of NT-proBNP,Scr,BUN and left ventricular end diastolic diameter(LVEDD)of the observation group were higher than those of the control group,and the differences were statistically significant(P<0.05).The left ventricular ejection fraction(LVEF)of(53.07±14.33)%in the observation group was lower than that of the normal group(68.61±9.28)%,and the difference was statistically significant(t=8.518,P<0.05);LVEDD of maintenance hemodialysis patients was positively correlated with NT-proBNP,Scr,and BUN levels(P<0.05),and LVEF was negatively correlated with NT-proBNP and BUN levels(P<0.01).Conclusion The systolic and diastolic functions of the heart of MHD patients are reduced,and the serum levels of NT-proBNP,Scr and BUN are increased.The changes in cardiac structure and function are related to the levels of NT-proBNP,Scr and BUN.MHD can improve patients with chronic renal failure prognosis,which provides ideas for reducing cardiac structure and dysfunction in patients with chronic renal failure.
