摘要
目的分析新生儿重症病房医院感染监测情况,并提出应对措施。方法选取2016年6月至2018年6月重症病房收治的356例新生儿作为研究对象,分析医院感染监测情况,观察重症病房新生儿医院感染分布部位、各体质量新生儿呼吸机和中央导管使用率及感染率。结果 NICU新生儿医院感染部位主要为下呼吸道,占比46.15%,其次为上呼吸道23.08%、皮肤15.38%、胃肠道7.69%、口腔7.69%;体质量≤1 000 g的新生儿医院感染率为35.71%,体质量1 001~1 500 g、1 501~2 000 g、>2 000 g新生儿医院感染率分别为4.08%、2.40%、0.84%,新生儿体质量越低,医院感染率越高;体质量≤1 000 g的新生儿呼吸机、中央导管感染率分别为10.71%、8.11%,体质量1 001~1 500 g的新生儿呼吸机、中央导管感染率分别为2.11%、1.92%,NICU新生儿呼吸机、中央导管感染率分别为3.03%、3.73%。结论临床应加强对NICU新生儿的医院感染监测,并及时采取适当措施干预,可积极预防或减少医院感染发生,保障新生儿健康。
Objective To analyze the surveillance of nosocomial infection in neonatal intensive care unit(NICU)and put forward countermeasures.Methods 356 newborns in ICU from June 2016 to June 2018 were selected as the research subjects.The monitoring of nosocomial infection was analyzed,and the distribution of nosocomial infection,the utilization rate of ventilator and central catheter and the infection rate of newborns with different body masst were observed.Results The main site of nosocomial infection in NICU was lower respiratory tract(46.15%),followed by upper respiratory tract(23.08%),skin(15.38%),gastrointestinal tract(7.69%),and oral cavity(7.69%);the nosocomial infection rate of newborns with mass≤1000 g was 35.71%,and the nosocomial infection rate of newborns with body mass of 1001-1500 g,1501-2000 g and>2000 g was 4.08%,2.40%and 0.84%,respectively.The lower the body mass,the higher the infection rate was,the infection rates of ventilator and central catheter were 10.71%and 8.11%for newborns with body weight≤1000 g,2.11%and 1.92%for newborns with body weight 1001-1500 g,and 3.03%and 3.73%for neonates with NICU.Conclusion In order to prevent or reduce the incidence of nosocomial infection and protect the health of neonates,we should strengthen the monitoring of nosocomial infection in NICU and take appropriate measures to intervene in time.
