摘要
目的探讨骨转化标志物在脊髓损伤患者中的表达特点,以及在跌倒相关骨折中的预测价值。方法收集111例脊髓损伤患者临床资料,抽取静脉血检测Ⅰ型胶原蛋白C末端交联肽、1型胶原蛋白氨基端前胶原肽、血清钙、维生素D3以及甲状旁腺素等骨转化生物标志物水平,随访1年内跌倒相关骨折发生情况,对比骨折患者和无骨折患者基线骨转化标志物水平差异,分析影响跌倒相关骨折的危险因素。111例患者在随访1年中发生跌倒相关性骨折18例,为骨折组,93例未发生骨折患者为非骨折组。结果2组在年龄、受伤时间以及步态上差异具有统计学意义(P<0.05)。与非骨折组相比,骨折组CTX水平明显增高(P<0.05)。经Logistic回归分析显示,跌倒相关骨折发生与CTX水平、年龄、受伤年限有关,与步态无关。经Spearman相关性分析显示,CTX水平与年龄和受伤年限呈负相关,与性别、BMI、损伤平面、步态无关。结论CTX水平增高可增加脊髓损伤患者短期跌倒相关的风险。
Objective To investigate the expression characteristics of bone transformation markers in patients with spinal cord injury and its predictive value in fall-related fractures.Methods To analyze risk factors for fall-related fractures,the clinical data of 111 patients with spinal cord injury were collected,Ⅰtype C terminal crosslinking peptide,type-1 procollagen N-terminal,serum calcium,vitamin D3 and parathyroid hormone level of transformation of bone biomarkers were detected,and the incidence of fall related fractures within 1 year was followed up.The baseline bone transformation markers were compared between patients with and without fracture.Results During the 1-year follow-up,18 of 111 patients had fall-related fractures(fracture group),and 93 patients had no fractures(non-fracture group).The differences in age,injury year and gait between the two groups were statistically significant(P<0.05).CTX level in the fracture group was significantly higher than that in the non-fracture group(P<0.05).Logistic regression analysis showed that the incidence of fall-related fractures was related to CTX level,age and injury years,but not to gait.Spearman correlation analysis showed that CTX level was negatively correlated with age and years of injury,but not with gender,BMI,injury plane,and gait.Conclusion Elevated CTX levels are associated with an increased risk of short-term falls in SCI patients.
